SCAN OF THE ABDOMEN AND PELVIS:- Multislice axial contiguous cuts were taken through the abdomen and pelvis with peroral gastrografin, and after IV. non- ionic contrast injection, in a triphasic fashion (arterial, portovenous and delayed phases). FINDINGS: — Massive free peritoneal fluid and air seen.Stomach and Bowel is unremarkable . Normal CT density of the liver parenchyma, with no focal lesions. _ No intra or extrahepatic biliary dilatation The portal vein is patent with no thrombosis. _ Normal CT appearance of the spleen and pancreas. Both kidneys are functioning and normal. . Normal filling of the urinary bladder with normal wall thickness and no calculi The prostate and both seminal vesicles are of normal C.T. appearance. No abdominal or pelvic lymphadenopathy. _ Bilateral pleural effusions seen IMPRESSIO| Free peritoneal fluid and air suggests ruptured viscous.Urgent surgical consult advised. Verified By Da. EARS Mz ‘The above named patient who is a known deaf and dumb presented with a diagnosis of an acute abdominal pain for one day and urgent surgical attention was taken where he underwent an emergency exploratory laparotomy and the intra- operation findings of a perforated disorder ulcer was made. Post operatively the patient had a permanent discharge on the incision site, sutures are released 2 then cleaning and dressing of the wound done then closed primarily and the patient was discharged. Upon discharge and considering the sound history of the patient, it is necessary to facilitate less distressful means of transport that is by air rather than by road.
