Case 2

Q1) what is the infection

bacterial infection because the culture growth is positive (bacteremia)

Q2) what do you think about the pathogen in blood and urine
Blood culture: the result indicate that the pathogen is staphylococcus. S. aures is the
most common agent cause bacteremia. but to insure and beeing more specific other test
must be done
1- coagulase test: if positive → the s. aures is the causative agent. If negative, test in
step(2) must be done
2- novobiocin test: if positive (sensitive)→ the causative agent is s. epidermidis. If the
test is negative→ s. saprophyticus is the causative agent

Urin culture: as the result the bacteria is B-hemolytic streptococcus but in this culture
we should count the growth
1) if there are more the 10 colony so 10000 cfy/ml, then their would be infection and
as I mentioned it will be b- hemolytic streptococcus (s. agalactiae and s. pyogen).
S. agalactia most common to cause uti but conformation test must be done. Camp
test must be done if it positive the causative bacteria is s. agalactiae. If negative s.
payogen will be the causative bacteria.
2) if the colonies count 1 or more but less than 10, meaning 1000-9000cfy/ml, there
is no significant growth because there is no clear uti symptoms (as in the case)
