I State that I have studied at Bhandarakars’ Arts & Science College, Kundapura affilated to Mangalore
University. I have misplaced my original BBM(Bachelor Of Business Management) degree Pass certificate
and Marks card while travelling by cab at Bangalore and I gave complaint at madiwala police station,
Bangalore on 23/03/2020 bearing complaint no 060811/2020.

I have tried searching it but could not find it, If in future I found the original I will surrender the same to
the concerned authority.

I state that, I want to obtain duplicate certificates from concerned authorities, hence this affidavit.

Wherefore, please issue duplicate certificate as mentioned above.
