2)Click on Corporate E-Card
3)Select Employee Id.
4)Slowly type Magna in group name
column & select Magna Infotech Pvt Ltd
from drop down
5) Enter your Magna Emp ID

48 hrs prior to hospitalization, reqest ins. desk at hospital to
Request Pre-Authorization send IRDA Preauth Form via mail to preauth@ghpltpa.com or fax
1860 425 4242 with some supporting reports received prior to
hospitalization.

 Good Heath will validate all documents.


Good Health may request for further information from hospital or
Obtaining Sanction treating dr or patient.
 Good Health will confirm / deny its decision to hospital
authorities depending upon your policy terms & conditions.

 Employee can confirm with hospital authority about the status


Confirm & Planing of your hospitalization request.
Hospitalization  Employee can log in and view the authorization in their
respective HR log in’s.
Reimbursement Hospitalization

Intimate to Good Health about


hospitalisation within 24 hrs via mail or
call.

Mail will be sent to employee with Claim


Control Number (CCN) and IRDA Claim
Form.

Collect all the documents received from


time of admission till time of discharge
from the hospital and submit the same
for processing.
Documents For Reimbursement

 Completely filled IRDA Claim Form.


 Original in detailed discharge summary.
 Original final bill with detailed break up from
time of admission till time of discharge and pre
numbered cash paid receipt.
 Incase of surgical package, detail break up of
package.
 Original investigation reports along with Dr’s
advice and payment receipt.
 Medicine bills with dr prescription.
 MLC / FIR in case of Road Transport Accident.
 Obestric history / GPLA STATUS and ante natal
scan report in case of maternity.
 Copy of patients Good Health Card, patients
photo id proof and employee id card.
 Original cancelled cheque leaf.

How to acess your login ?

1) Log on to www.goodhealthtpa.com

2) Under Web Login Option, click on “More”

3) Select “Corporate Employee Login.

4) Click on “Don't Know Good Health ID” and log in with


Emp.ID.

5) Select Group Name as “Magna Infotech”

6) Enter your Emp.Id.

7) By default password is “1234”

8) Now you can access all your mediclaim details.


Points to remember

1) Hospitalization intimation should be given to Good Health Insurance TPA within


24hrs. Failing which claim will be referred to your insurer. Mail should be sent to
aloksingh.b@ghpltpa.com or SMS on 9849950462.

2) After discharge, within 07 working days, all original hards documents as


per check list and claim form should be courierd to :
Plot No. 49, Nagarjuna Hills, Panjagutta, Hyderabad. Telangana – 500 082.
Addressing to Alok Singh and should mention Magna Emp ID.

3) For any medi claim issue, mails should be marked to


aloksingh.b@ghpltpa.com and can be reached on 9849950462.

4) For any follow up of cashless claim, please provide Ticket ID received by


hospital.

5) All your medi claim details can be accessed via mobile app “GetSetGo” on
Android Phones.
Standard Policy Exclusions

1) Dental treatment not covered, unless requring hospitalization.

2) Cosmetic / obesity related treatments not covered.

3) Alcoholic / drugs related treatments are not covered.

4) Congenital external treatment not covered.


5) HIV related treatments not covered.
