O – Patient rates pain 9/10 (1 is the lowest and 10 is the highest Vital signs of T-36.4
BP-110/70 PR-76bpm RR-22cpm

P – after 4 hours of nursing intervention client will show comfort and ease in every
move she does

I - Promote perineal exercise and sitting position such as Kegel’s exercise,

Recommend sitting in warm bath (sitz bath Apply cold packs or heat packs on the
stitches. And will tell the patient that pain will only last for a few days.

E - After hours of nursing intervention the patient was able to experience lesser pain
and above tolerable level showing comfort and ease in moving.
