AUTHORITY TO DEDUCT FOR LOSSES/SHORTAGES/THEFT

To the Payroll Master:

I hereby authorize Human Resource Solutions, Inc. to deduct from my salary and/or
deposit the amount of losses/shortages/theft of inventory/stocks/goods in my custody that
would be determined after due internal investigation and audit.

I confirm that the term “salary” herein shall include approved overtime pay, validly
monetized leaves, allowances due, eligible bonuses and 13th month pay, and other similar
forms of monetary compensation.

I understand and agree that I am responsible for satisfying the above amount of
losses/shortages/theft.

I understand and agree that any amount of losses/shortages/theft of

inventory/stocks/goods that is due and owing at the time of my termination, regardless of
whether my termination was voluntary or not, will be deducted from my last pay.

In the event a new Authority to Deduct for Losses/Shortages/Theft is not executed on or

before the end of the year, this form shall be deemed to continue in force for the
succeeding year.
