CONTRACT AGREEMENT FORM

This Contract Agreement is made and entered on ______ by and between;

XXX, with its principal office address at Metro Manila, herein referred to as the FIRST
PARTY;

YYY, with business address at Bonifacio Global City, Fort Bonifacio City of Taguig,
herein referred to as the SECOND PARTY.

WHEREAS, the SECOND PARTY stated the following scope of work of the consultant
on the Terms of Reference for the service needed;

NOW THIS AGREEMENT WITNESSETH AS FOLLOWS:

1. The SECOND PARTY shall prepare and submit the following Expected outputs /
deliverables:

2. In consideration of the payments to be made by the FIRST PARTY to the SECOND

PARTY, the SECOND PARTY hereby assures to provide the goods and services
and to remedy defects therein in conformity in all respects with the provisions of
the Contract;

3. In case of any dispute that may arise in connection with this agreement, the Parties
shall exert best efforts towards an amicable settlement. Otherwise, any case arising
from this contract shall be exclusively brought to the proper courts of Taguig City
to the exclusion of all other venues;
