AUTHORIZATION FOR USE OF IMAGE AND PRIVACY STATEMENT

I hereby irrevocably consent to and authorize the use by MONASHEES GESTÃO DE

INVESTIMENTOS LTDA., a Brazilian limited liability company, with head offices in the City and
State of São Paulo, at Rua Samuel Morse 74, cj 173, enrolled under CNPJ number
06.130.952/0001-06, and its Affiliates (“Monashees”), to capture and record my image, audio,
comments and other personal data related to my participation at the “monashees 2021 music
video ” recorded on “2021” to then be stored and used.

I consent to such Data processing by Monashees to allow: (i) free distribution and external and
internal sharing of the recording that contains my Data with Monashees stakeholders in Brazil
and abroad, for the purpose of institutional promotion of Monashees, in all medias, whether
now known or hereafter created, throughout the world in perpetuity; (ii) storing of the
recording that contacts my Data.

I understand that I have the right, as data subject, to access data relating to me, to be informed
about the existence and the extent of data processing and to rectify incorrect personal data as
the case may be. The personal details contained in the present consent form will be kept as
proof of your consent. You can freely withdraw your consent at any time by using the contact
details mentioned herein. In that case, all processing operations that were based on your
consent and took place before the withdrawal of consent remain valid.

If you do not agree with your image or voice being recorded and published, please inform
Monashees. In this case, and depending on available resources, Monashees may offer
alternatives.
