certificate bearing certificate number 90519131103971 which issued on 10th July 2014.

Through this letter, I would like to request you to kindly correct my father’s name on the caste
certificate. In my all-educational documents the name of my father is Kailash Chander but the name
mentioned on my caste certificate i.e., Kailash Chander Meena is incorrect. Kindly find the below-
mentioned correct spelling of my father’s name along with the required information.

Respected Sir/ Madam,

I, Raj Kumar, S/o Kailash Chander writing this letter to request you for name correction in caste
certificate bearing certificate number 90519131103971 which issued on 10th July 2014.

Through this letter, I would like to request you to kindly verify my caste in my caste certificate and
reprint it in a new format. My caste is Scheduled Tribe, but on the issued certificate the caste is
mentioned in old format of SC/ST certificate which not clearly expressed my cast category.

And, my father’s name mentioned on my caste certificate and is Kailash Chander Meena, but on my
educational document my father is Kailash Chander, so kindly ignore it or remove surname of my
father from my caste certificate. I am enclosing all supportive documents required for correction in
the certificate.

my father’s name or remove my father surname in my caste certificate (ST) instead of Kailash
Chander Meena to Kailash Chander, bearing certificate number 90519131103971, because my all-
educational documents and other documents my father’s name is mentioned as KAILASH CHANDER.
through this latter I would like to request you to kindly correct my father’s name in my caste
certificate.

I have applied for caste certificate in the year of 2014 with the name mention from my father is
Kailash Chander and said certificate issued on 10-07-2014 but the name mention in caste certificate
is incorrect. It is also informed that my father’s name is KAILASH CHANDER MEENA is mentioned in
all documents of my brother. I am enclosing all supporting documents required for correction in the
same.
