Attestation Template for Insurance Advisers

I, Name of Agent attest that I assisted <Client’s Name> with his/her request for <<Type of
Request>> and I:

• secured the consent of the Client/Policy Owner for purposes of this request/transaction;

• explained to the client the needed information to accomplish the request relative
to Policy No. __________________, and that any information I have entered into
the form are based on data and/or documents that the customer provided to me,
and that I performed such upon the express consent, and on behalf, of the client;

• conducted the Know-Your-Customer and Client Due Diligence procedures required by

the Anti-Money Laundering Act and the Terrorist Financing law, and took a screenshot
of our videoconferencing;

• confirm that the Client/Policy Owner is alive at the time of submission of the request,
represented to me that he/she is in the Philippines, and a visual match with the face
appearing in the identity document the Policy Owner has submitted.

• confirm that the Policy Owner is the one paying premium and there is no third party
payor (for transactions with premium payment of above Php500,000).

I have used my Manulife email address in transmitting documents to the Client and to the
Company. Any hard/paper copy of the Client’s product application/documents in my possession
has been safely and securely destroyed in accordance with the Company’s policies.
