Initial Investigation Report re-Fire incident
2. This pertains to the Fire Accident Transpired along Edsa, Brgy 123 Metro Manila at
about afternoon of May. 6, 2021 which smoked tire

3. The incident was reported by a concerned people at about on afternoon of the same
date thru mobile phone call which was received by Manila Police DistrictPS
INFORMING that a fire incident transpired at the same place immediately, a team led by
the undersigned proceeded to the reported place

4. Initial investigation conducted disclosed at about 12:42pm of May. 6, 2021 a RISING

FROM THE INTERIOR saw smoke Boni MRT station and 2 suddenly boy seen, I used
my department cell phone to take a picture of Dela Cruz and of Malakas. I then called
headquarters and spoke to Detective Jimmy Santos. I gave her the boys’ names and
addresses for further investigation. I drove back to the Parking lot, parked my patrol car,
and talked to firefighter Domingo Mendez, who told me the fire had already been
extinguished.I returned to the station, downloaded and printed the pictures of Dela Cruz
and Malakas, and gave them to Detective Santos
