Parent Questionnaire

This survey is to help me understand where you believe John is at this point of the year.
Please fill this out as best you can, and if you have any questions, please contact me!

Background:

1. Student Name: __John Fitzgerald___________________________

2. Name of parent/guardian filling out form: _Aimee Fitzgerald_______

3. Birthplace of student: ___Baton Rouge, LA___________________________

4. Were you aware of anything your child struggled in before 1st grade? (Disabilities,

hard subjects, etc.): YES / NO

General:

1. How do you think your child has liked 1st grade so far? ___He enjoys it, but doesn’t

LOVE school. He does like his teachers and friends.

2. Do you have any concerns about your child at this point in the year? (Socially,

academically, etc.): __No. We do have some emotional struggles, but I don’t think
that’s an issue at school. He is also very quiet in what he needs; not using words

like he should. But once again, I don’t think this is a struggle at school.

3. Any strong-suits you have noticed in your child for 1st grade? __He is doing really

well with his reading. He seems to catch on quickly. He can struggle in the

evenings being tired and cranky, but when he wants to read, he does great.

4. What are your child’s likes and dislikes about the school year thus far: _He likes

ancillary and recess. He doesn’t like Wit and Wisdom. He says it’s too long. He

said that in Kindergarten as well, so I’m not sure if it’s just the words “Wit and

Wisdom” or if he really doesn’t like the info being presented.

5. Is there anything you would like your child to have more support with? _Not

academically. I wouldn’t mind learning more or having more guidance with

emotional support in young children._

6. As my time is wrapping up at Mayfair, is there anything else you would like me to

know about your child or in general? _He wants you to know he lost 4 teeth ;) He’s

very loving and kindhearted, especially to others that aren’t his brothers— and

especially babies/toddlers. He holds his emotions close and is the toughest of my 3

boys. He love to build, color, play sports, and wrestle.
