INCIDENT DESCRIPTION

10 in the evening John Karl T. Nacion with Jean B. Laryona travels a road to area of

home,at Sinawal Apopong General Santos City few minutes in the left side they have a car

driving so past along to my position of my car and he hit me and after that I Did not

know because I Collapse , later on when I Woke up I was in the hospital and I Found out

my girlfriend is gone.
