1. THAT, my maiden name is Carren Joy Bitalac Otivilladar as stated and registered in
my birth issued by Office of the Civil Registry Certificate. A copy of my birth certificate
is hereto attached;
2. THAT, in my previous filing in PAGIBIG, my middle name was erroneously written as
“Betalac” instead of my true and correct middle name “Bitalac”;
3. THAT, my true middle name is “Betalac”, as appearing in the Certificate of Live Birth as
the maiden name of my mother;
4. THAT, in all pertinent documents which I had used, to include that of my scholastic
records (transcript of records). I used my true and correct middle name of “Bitalac”;
5. THAT, I am executing this Certificate of Oneness, to attest the truth of the foregoing
facts as regards my true middle name.
