GUIDE: DICT Digital Signature Application Form

*** Must open Google Chrome browser on your desktop computer or laptop ***
1. Go to the website of Saint Anthony Mother and Child Hospital (https://samch.doh.gov.ph/)
2. Click “DICT Digital Certificate Application Form”. as shown below:

4. Follow instructions of the PNPKI Application for Digital Certificate when filling the form:
1. Please fill the form in BLOCK LETTERS ONLY.
2. Required fields are marked with an asterisk (*). (also highlighted in RED BOX)
 Fill the First Page only and fill other field not highlighted in red box as much as possible
3. Present one (1) copy and the original documents to our Registration Authority for verification and attestation.
4. Any information that is not verified shall not be included in certificates.
5. Any discrepancy or inconsistency in the form will lead to delay and/or rejection.

5. After filling all the Fields, hover your mouse on the upper left corner until the Printer Icon is displayed
and click the Printer Icon. as shown below:

6. Choose “Save as PDF” from Destination. as shown below:

7. Then click “Save”. as shown below:

8. “Save As” box will appear. Click “Desktop” to save you file to desktop for easy retrieval.
9. Then click “Save”. as shown below:

10. Locate your saved PDF File from the Desktop of your computer and affix your signature before
submitting it to I.T. Office.
11. You may also copy and save the PDF File on your USB Flash Drive to have SAMCH I.T. Personnel
print the file and affix your signature before submitting it.
