Dear Sir/Madam,

Through this letter, In connection with my health condition does not allow, and therefore I
would like to apply for early retirement.

I also express my gratitude to the Company, PT. Kurita Indonesia for the support and the
opportunity that I have gained over almost 24 years. I do hope in the future I can continue to
work and flourish in other field. I do apologize if my actions were less pleasing during the
work. Hopefully the company has always continuous to grow and developing
