Voluntary Student Onboarding Consent Form

I , student, roll number of the program

Batch , hereby declare the following:

a) I understand that the purpose of the campus on-boarding is to facilitate the Academic
Rigour that can enhance the learning experience and deduce the limitations due to the
COVID pandemic. I am responsible to maintain Academic Discipline while adhering
to the guidelines stipulated by the Programs Office.

b) I will adhere to the COVID safety guidelines/protocol stipulated by the Institute and
concerned government authorities without fail including any amendment done from
time to time.
c) I will go through all the tests/precautionary measures such as RT-PCR, COVID
vaccination as per the instructions of the Institute/Government authorities.
d) I will adhere to the instructions given by the concerned medical authorities concerning
the diagnosis, medication, treatment, and hospitalization considering the COVID
pandemic situation that includes the admission/discharge to/from any medical facility
at the discretion of the concerned medical experts.
e) I will adhere to all the disciplinary rules and regulations stipulated by the Institute.
f) I will adhere to the room allocation policy and will be willing to move to any allotted
executive/non-executive hostel room with a single/shared washroom as instructed by
the concerned authorities anytime during my tenure at the Institute for changes if any
during the COVID pandemic circumstances.
g) I will adhere to the stipulated policies with regards to the facilities (such as air-
conditioning, gym, sports, music, etc.,) that can be amended from time to time at the
discretion of the Institute.
h) I am voluntarily making a conscious choice to on-board, and the Institute will not be
held responsible for any unforeseen circumstances as I understand that the risk
involved due to the COVID pandemic.
i) I hereby declare that my health is apt for onboarding and do not have any concerning
medical history/allergy (if any, email student.relations@iimu.ac.in).
j) I take the responsibility to undergo both first and second dose of COVID vaccination
as per the government regulation and incur any cost involved for the same
I hereby declare to adhere to all the terms and conditions mentioned above and any
amendments in the future. I am aware that I am bound to vacate the campus in case of
any violation or as and when instructed by the Disciplinary Committee of the Institute.

Parent/Guardian Consent Form

I , parent/guardian is aware that my ward
Mr/Ms. has opted to onboard during the COVID pandemic
situation and will co-operate and adhere to all the instructions/guidelines/protocol stipulated
by the Institute/concern Government authorities. I shall respect the decision of the
management at any given point of time for the well-being of my ward during the tenure at
IIM Udaipur.
