JOINT AFFIDAVIT OF ATTACHMENT OF SIDECAR

We, JOHN JUAN RONQUILLO of legal age, Filipino, and a resident of

B34 L18 P2 Sicily St., Calamba Hills Village, Brgy. Barandal, Calamba City,
Laguna; and
HARRY DELOS SANTOS TIDON of legal age, Filipino, and a resident of
408 Maligaya Village Lawa Calamba City, Laguna, after having been duly sworn
to in accordance with law, do hereby depose and say:

That, JOHN JUAN RONQUILLO is the owner of one (1) unit motorcycle
more particularly described as follows:

MV FILE No. : 0422-00000096193
Plate No. : 1784WY

That, currently said motorcycle is registered with a denomination of

WITHOUT SIDE CAR and that JOHN JUAN RONQUILLO have and/or want to
attach a sidecar to said motorcycle;

That in order to attach said side car the services of HARRY’S WELDING
SHOP located at Maligaya Subd., Brgy., Lawa, Calamba City, Laguna owned by
HARRY DELOS SANTOS TIDON, who was the mechanic who serviced the
motorcycle and had attached the side car thereunto was the one contracted to
render said service;

That we are executing this affidavit to change the status of registration

from WITHOUT SIDE CAR to WITH SIDE CAR and to attest to the truth of the
above statement.
