Monday September 4:

Labor Day Holiday.

Tuesday September 5:

 Safety meeting JSA for Hansa Meyers Crane Operations and Lonestar Crane Operations.
 Material Disposition log is being updated daily.
 Lift Report is updated daily as new equipment and modules are being rig down.
 Continue disconnecting and rolling back power cables on both sides of module 0030B (drag
chain)
 Compile work packs for the rig down activity files.
 Prep documents for further rig down sign offs.
 Continue conversion of the marked up drawings, photographs, work instructions, etc. for Rig Up
manual for use on site.
 Lonestar crew spend most of day taking out pins on module 9010B
 Identified walkway pieces that were not labeled.
 Working on module work packs for items completed at site.
 Rig down the following: Module 4210 and 9010 West crane base
 Dave Ellis visit GDS location where the top drive is being assessed/examined.

Wednesday September 6:

 Safety meeting JSA for Hansa Meyers Crane Operations and Lonestar Crane Operations.
 Material Disposition log is being updated daily.
 Lift Report is updated daily as new equipment and modules are being rig down.
 Resumed disconnecting cables from M3320 Gen # 3 module to M4320 module. 100% completed
 Continued disconnecting cables from module M8010 stairway tower. 100% completed. Module
M8010 Ready for lift.
 Continued disconnecting 600V high voltage cables from north end of M0030B drag chain. Pulled
cables back to cable tray on M4120 module. 50% completed
 Compile work packs for the rig down activity files.
 Prep documents for further rig down sign offs.
 Continue conversion of the marked up drawings, photographs, work instructions, etc. for Rig Up
manual for use on site.
 Mechanical- at LSEF
 9020B East lower crane base stair way removed. 09:08hrs.
 Lonestar crew removing pins from upper DES modules and brackets.
 Walk through and signoff by Wavecrest, ABS, and Lonestar of modules

Top Drive Repairs at GDS Int’l.


 Shipping Skid/Lower Guide Track Section
 Skid is removed and in inspection for damages.
 Cage Repairs
 Cage is in the weld shop and repairs are 75% complete.
 Hydraulic Leak Test of Gear Box
 GDS is in the process of performing hydraulic leak test of the gear box/hydraulic system. The
idea of the leak test is to confirm all internal seals are in working condition prior to the top drive
entering the test stand for run test (i.e. – avoid dual efforts).
 Removal of Bail Pin Retainer Blocks & sheared bolt
 GDS believes they can remove the “wedged” bail pin retainer blocks as well as the associated
sheared bolts without removal of the top drive from guide dolly frame. If they are able to do so
this we will save considerable hours of labor. This effort is in process and will be updated.

Thursday September 7:

 Safety meeting JSA for Hansa Meyers Crane Operations and Lonestar Crane Operations.
 Material Disposition log is being updated daily.
 Lift Report is updated daily as new equipment and modules are being rig down.
 Electrical- At LSEF
 Continued disconnecting cables on module 0030B drag chain to cable tray in module 4120.
Cables were rolled back and secured on both ends. (100% Completed).
 Witnessed completion of lifts on mud pumps 1-3. Mud pumps were staged on west end of rig.
 Continued to update electrical work packs
 Compile work packs for the rig down activity files.
 Assist in development of the Module Installation Sequence.
 Continue conversion of the marked up drawings, photographs, work instructions, etc. for Rig Up
manual for use on site.
 Lonestar removing pins and prepping Mud pumps #3, #2, #1 and modules M3110 and 3120 for
removal.
 Working on module work packs for items completed at site.
 Working on Rig Up manual.
 Rig down the following: Mud Pump inside Ladder 1, Mud Pump inside Ladder 2, Mud Pump

Top Drive Repairs at GDS Int’l.


Shipping Skid/Lower Guide Track Section
 Lower Guide Track section has been inspected and is damaged in one bracing section.
Measurements are to be taken to determine if a repair is necessary.

Cage Repairs
 Cage is repaired and ready for paint.

Hydraulic Leak Test of Gear Box


 Leak test continues with diesel in place of oil in order to find any seal leakage.

Removal of Bail Pin Retainer Blocks & sheared bolts


 Contrary to yesterday’s hopeful report it was found necessary to remove the top drive from the
trolley in order to access the bail pin retainer blocks. Those are where the sheared bolts were
that everyone noticed after the incident. We had hoped we could get them out without taking the
TDS off the trolley. Today partial disconnection of cables and fold back of the j-boxes and hoses
commenced in order to free up the top drive.

Friday September 8:

 Rig down the following:

 Shipped out following Platforms, Walkways, or Modules:
o No Shipments
 Yard conducted a Safety Stand-down due to incident 7 Sep, 2017.

Saturday September 9

 Material Disposition log is being updated daily.


 Lift Report is updated daily as new equipment and modules are being rig down.
 Walk modules to verify pipe disconnects made and organize pipe spools for packing and
shipment.
 Continued to update and complete electrical and mechanical rig down work packs.
 Continued formatting Rig up Manual and compiling work pack documents.
 Mammoet cranes commenced to be rigged up.
 Due to ongoing (Mammoet) rigging operations and Lonestar operations, electricians were unable
to use man lifts to work on connecting and disconnecting instruments on cranes and crown.
 Rig down the following:
o No lifts
 Shipped out following Living Quarters:
o No Shipments
 Electrical is completely disconnected on Rig.

Sunday September 10:

 Painters and welders continue with repairs and touch ups.
