AFFIDAVIT OF DISCREPANCY

(ONE AND THE SAME PERSON)

I, JOSEPHINE MADLANGSAKAY y GOZON, Filipino citizen, married, of

legal age, with address at 132 P. MONTOYA ST., BRGY. 5, SILANG, CAVITE,
after having been duly sworn to in accordance with law hereby depose and
state:
1. My maiden name is JOSEPHINE ENCABO GOZON, as evidenced by
my Certificate of Live Birth bearing the name of my father
DOMINGO L. GOZON, and my Certificate of Marriage also indicating
my father’s name.
2. Sometime in _______, after I graduated from college, I discovered that I
did not have birth certificate. At the same time, I discovered that my
last is GOZON, which prompted me to personally file for DELAYED
REGISTRATION OF BIRTH with the Office of the Civil Registrar of
Silang, Cavite.

4. On the other hand, the Office of the Municipal Civil Registrar of Mati,
Province of Davao Oriental erroneously recorded my name as Evelyn
Kumaingking Kumahig as appeared on the Register of Births.
5. I have used my maiden middle name with “Co” as in Comaingking
instead of Kumainging and my maiden last name with “Co” as in
Comahig instead of “Kumahig” in all of my legal, official and public
records and documents.
6. I, EVELYN COMAHIG MADLANGSAKAY and Evelyn Kumaingking
Kumahig are one and the same person.
7. I am executing this affidavit to attest to the veracity of the foregoing
and for whatever legal purposes in may serve.
