Whereas, Chief Medical Officer Kupwara advertised 18 posts of registered
Pharmacists to be hired on Profit Sharing basis under Pradhan Mantri Bhartiya Jan Aushadhi
Kendra (PMBJAK) for 09 Health facilities i.e. CHC Sogam, CHC Kralpora, CHC
Zachaldara, CHC Langate, CHC Kralgund PHC Kalaroose, PHC Trehgam, PHC Villgam &
PHC Chogal under DIPK No: 11737/21
Whereas, interview notice for the selection of candidates was issued vide Notification
No: CMOK/NHM/2021/1600-03 dated: 17-11-2021 under DIPK No: 15372/21 and interview
was conducted on 24th of December 2021.
Whereas on the basis of merit and having other requisite eligibility Criteria, the
candidates mentioned below are hereby provisionally selected against the posts of registered
Pharmacists (Jan Aushadhi Store Manager) under Pradhan Mantri Bhartiya Jan Aushadhi
Kendra (PMBJAK) for 09 Health facilities i.e. CHC Sogam, CHC Kralpora, CHC
Zachaldara, CHC Langate, CHC Kralgund PHC Kalaroose, PHC Trehgam, PHC Villgam &
PHC Chogal.
Whereas, the selection is provisional & selected candidate are informed to join along
with all original relevant testimonials/documents in the Office of Chief Medical Officer
Kupwara within Seven (07) days from the date of publication of this notification, failing
which his/her selection shall be cancelled without any notice.
Whereas The Selected Pharmacists have to cancel their license issued in FORM 20/21
before joining under PMBJAK in the office of undersigned.

The Selection is subject to the verification of all relevant documents. The selected
candidates shall forfeit his/her claim to the post if at any stage it is found that he/she has
misrepresented the information or produced invalid documents, fresh applications shall be
invited as per the prescribed procedure.

In addition to that other terms and conditions shall remain as per the affidavit/
agreement dully executed by the engage with the chief medical officer Kupwara in the court
of law and as per the terms and conditions depicted in Advertisement Notice No. and date
referred above and as per the guidelines of the PMBJAK programme.
