Introduction
Chapter 1: Introduction
Overview
Anesthesia is a medically-induced temporary state of unconsciousness during which the recipient is
unaware of and insensitive to pain. Anesthetics are administered prior to and during surgical and dental
procedures. The use of anesthesia can suppress a patient’s central nervous system and alleviate anxiety
and inhibit long term memories of what can be very painful and traumatic procedures. The use of
anesthesia not only prevents and manages pain, it also enables less traumatic, faster operations with
fewer complications to the patient.
Anesthetic devices are used to safely deliver a measured quantity of medical gases that are mixed with a
specific concentration of anesthetic vapor during surgical procedures.
Artificial respiration or aided respiration is a mandatory healthcare practice for patients suffering from
severe respiratory diseases, critical conditions and traumatic injuries. Respiratory devices include those
devices which measure respiratory function as well as those which primarily eliminate mucus from
respiratory airways; this leads to the improvement of the pulmonary function.
There is a considerable increase in the number of operative conditions or surgeries globally resulting in a
significant opportunity for the anesthesia devices market to grow. Increases in air pollution along with
more diagnosed respiratory infections, allergies and diseases such as Chronic Obstructive Pulmonary
Disease (COPD) and Obstructive Sleep Apnea (OSA), are factors impacting the usage of respiratory
devices.
The anesthesia and respiratory devices market covers airway and anesthesia devices that are used in

Chapter 3: Overview of Anesthesia and
Respiratory Devices
Anesthesia Devices
Anesthesia device are those devices that assist anesthesiologists in accurately administering anesthetic
gases or vapors in a desired quantity and concentration. The adoption level of these devices is currently
higher in developed nations than in developing nations. There are various reasons for this difference
including the lack of regulations, a gap in available technology, doctors’ preferences towards
conventional procedures and economic constraints. However, because of some significant changes in
the healthcare policies in some Asian and Latin American countries, the usage of these devices is
expected to grow in the future.
Respiratory Devices
Respiratory devices are those devices that provide artificial respiration for a patient who is unable to
breathe regularly without assistance. Generally, artificial respiration is provided to patients who suffer
from chronic respiratory issues, are critically ill, are admitted in a neonatal care unit, or are placed in an
intensive care unit for enhanced respiratory support. These devices play a significant role is supporting a
patient’s recovery. These devices are generally of two types, hospital-based and home-based.
Laryngeal Masks
A laryngeal mask is a device that is used to maintain an effective airway without tracheal intubation. It
consists of an inflatable silicon mask and a rubber connecting tube. It is inserted into the pharynx,
forming a low-pressure seal around the laryngeal inlet and permitting gentle positive pressure
ventilation.

Tracheostomy Tubes
Tracheostomy tubes are meant to support airflow and help the patient breath. The surgical procedure of
inserting a tracheostomy-tube into a patient’s windpipe is called tracheotomy. It is a 2-to-3-inch-long
curved tube designed to be directly placed into a patient’s windpipe through neck.
Endotracheal Tubes
An endotracheal tube is a breathing tube that is used temporarily to keep the airway open so breathing
can occur. This curved tube is inserted into the patient’s nose or mouth into his traches (windpipe). Tape
or a soft strap holds the tube in place. Size range from 3-10.5mm in internal diameter.
Anesthesia Circuits
An anesthesia circuit is an interface between an anesthesia machine and patient and is used to deliver
anesthesia to the patient. A typical circuit consists of a reservoir, rebreathing bags, flexible hoses,
absorbers, and a face piece.
Non-Breathing Circuits
Non-Breathing circuits provide patients access to environmental air as well as the anesthetic oxygen
mixture and allows exhalation into the environment. It is a simple anesthesia path which consists of a
mask worn by the patient.
Rebreathing Circuits
A rebreathing circuit promotes the recirculation of anesthetic gases. It consists of a carbon dioxide (CO2)
absorber, a reservoir and two uni-directional valves, one expiratory and one inspiratory tube.
Anesthesia Masks
An anesthesia mask includes a mask for providing anesthesia to the patient, a mask strap for holding the
mask against the patient’s mouth, and a jaw raising strap to raise the patient’s jaw and forward to avoid
airway obstruction.
Breathing Filters
Breathing filters are single use disposable heat moisture exchange filters used in anesthesia and critical
care breathing circuits when it is very important to keep filtered and humidified anesthetic gases
inspired by the patients. This segment includes either bacterial filters, HME filters or a combination of
both.

to displace the lower pharynx, larynx and upper trachea. It consists of a spatula to hold the tongue, a
curved or an angular flange to provide rigidity with an L-shaped or semi-circular groove for viewing
along its length and usually an illumination system using a conventional lamp or fiberoptic system.
Laryngoscope blades are available in various sizes for adult and pediatric use; they are also available as a
non-foldable integral part of plastic laryngoscopes. Some laryngoscopes are designed specifically for use
in a magnetic resonance imaging (MRI) environment. Disposable and reusable laryngoscope blades are
covered under this segment.
Laryngoscope Handles
Laryngoscope handles are designed to be handheld while also suppling power to the laryngoscope
blades. It usually consists of a hollow handle with a battery and battery charger. The handle and an
attached standard, conventional blade compose a working laryngoscope. Dedicated fiberoptic
laryngoscope handles are also available. MRI compatible laryngoscope blades are made of non-
magnetitic materials so that they can be safely used in MRI suites. Disposable and reusable laryngoscope
blades are tracked in this segment.

Respiratory Devices
Respiratory devices are the devices that facilitate breathing during respiratory failure and other
respiratory disease conditions. There are many types of respiratory devices available in the market.
The following types are covered as a part of this report positive airway pressure (PAP) devices,
humidifiers, nebulizers, oxygen concentrators, oxygen conservers, reusable resuscitators and
ventilators.
Humidifiers
A respiratory gas humidifier ensures that patients on mechanical ventilation are supplied with optimally
conditioned respiratory gas. In active humidifying processes, moisture and heat is input to respiratory
gas by an electrically powered humidifier. The aggregation of water in the gas produced by an active
respiratory gas humidifier may be a suspension, or aerosol, which is produced by a nebulizer;
or particulate water, output from an evaporator or bubble humidifier.
Nebulizers
Nebulizers generate aerosols consisting of droplets of various sizes that are admixed to the inspired
respiratory gas. Types of nebulizers currently on the market include portable compressor nebulizers,
portable ultrasonic nebulizers, standalone compressor nebulizers and standalone ultrasonic nebulizers.

Oxygen Concentrators
Oxygen concentrators are used to deliver oxygen to those who require it. Patients may require it if they
have a condition that causes or results in low levels of oxygen in their blood. Several parts make up a
concentrator, including a compressor, sieve bed filter and circuit boards. The working principle of an
oxygen concentrator is much like a window air conditioning unit: it takes in air, modifies it and delivers it
in a new form. An oxygen concentrator takes in air, purifies and delivers it for use by people requiring
medical oxygen because of low oxygen levels in their blood. Oxygen concentrators are of two types:
fixed oxygen concentrators and portable oxygen concentrators.
Oxygen Conservers
Oxygen conservers regulate the flow of oxygen from an oxygen tank. They deliver oxygen as the patient
inhales, resulting in greater efficiency and cost saving ratios. Oxygen conservers are most commonly
utilized with home oxygen delivery systems and come in a variety of sizes and configurations to fit
different oxygen tanks and cylinders.

Continuous Positive Airway Pressure (CPAP) Devices
Continuous positive airway pressure (CPAP) machines are used for the treatment of sleep apnea and in
intensive care units as a form of ventilation. The machine delivers air at a titrated pressure determined
by the sleep physician. It consists of an airflow generator, an interface (nasal pillow, nasal or full-face
mask, or most commonly a lip seal mouthpiece) that provides connection to the user’s airway and a
hose connecting the flow generator to the interface.
Reusable Resuscitators
A reusable resuscitator is a device in which positive pressure is used to inflate the lungs of an
unconscious patient who is unable to breath, keeping them oxygenated and alive. It is operated through
a regulator which delivers gas through pressure. The regulator can be either automatic or manual.
These are manufactured from silicon and are latex free.
Ventilators
A medical ventilator is a device meant to provide mechanical ventilation to a patient. A positive
pressured ventilator consists of a compressible air reservoir or turbine, air and oxygen supplies, a set of
valves and tubes and a disposable or reusable patient circuit. Based on usage they are categorised as
critical care ventilators, neonatal/paediatric ventilators and transport ventilators.
Neonatal/Pediatric Ventilators
Neonatal/pediatric ventilators are the devices those provide artificial respiratory support to pediatric
and neonatal patients. They are specially designed for pediatric use. It consists of a compressible air
reservoir or turbine, air and oxygen supplies, a set of valves and tubes and a disposable or reusable
patient circuit.
Transport Ventilators
Transport ventilators are used to provide ventilation or artificial respiration to critical patients during
their transportation between various departments of a hospital or outside the hospital in mobile
medical transport applications. It consists of a compressible air reservoir or turbine, air and oxygen
supplies, a set of valves and tubes and a disposable or reusable patient circuit.

respiratory assistance. This device consists of a plastic tube which fits behind the ears, and a set of two
prongs which are placed in the nostrils. Each cannula consists of an open recessed channel disposed
between a pair of bridges for retaining extension tube within the ear piece and a locking exit hole at its
distal end for helping to secure the extension tube to provide a fixed distance adjustment between the
nasal cannula and ears of the patient.
Oxygen Masks
An oxygen mask is a mask which is placed on the mouth and nose and through which oxygen is supplied
from an attached storage tank to the lungs. Oxygen masks may cover the nose and mouth (oral nasal
mask) or the entire face (full face mask). They may be made of plastic, silicone or rubber.
Patient Interface
A patient interface is a mask used together with a positive air pressure device or a home ventilator,
worn by a patient during the time the positive pressure or ventilation therapy is delivered. This includes
nasal mask, nasal pillows and full-face mask.

Chapter 4: Market Dynamics
Market Drivers
Increasing Prevalence of COPD
Chronic obstructive pulmonary disease (COPD) is a lung disease characterized by chronic obstruction of
lung airflow that interferes with normal breathing and is not fully reversible. COPD is a term that is used
to include chronic bronchitis, emphysema, or a combination of both conditions.
Chronic Bronchitis
Chronic bronchitis is a condition leading to swollen breathing tubes (airways) because of mucus (phlegm
or sputum) production. Airway obstruction occurs in chronic bronchitis because the inflammation and
the excessive production of mucus leads to the narrowing of the breathing tubes. In cases of
emphysema, the alveoli walls are damaged; the alveoli lose their stretchiness and ability to trap air and
result in difficulty in pushing all the air out of the lungs. Because the lungs do not empty efficiently, the
lungs contain more air than normal; this is called air trapping and causes hyperinflation in the lungs.
Having extra air in the lungs and the excess mucus on a consistent basis causes constriction or
obstruction and makes the interior of the airways smaller than normal.
Diagnosis- The diagnosis of chronic bronchitis is made based on symptoms of a cough that produces
mucus or phlegm on most days, for three months, and in some cases the cough is observed for two or
more years (even after various treatments for cough reduction).
Emphysema
Emphysema is a lung condition in which the walls of the air sacs (alveoli) are partially or completely
damaged. A healthy human lung consists of more than 300 million alveoli which are normally stretchy
and springy, like small balloons. Same as a balloon, it takes effort to blow up normal alveoli and it takes
no energy to empty normal alveoli. When alveoli are unhealthy and have lost the stretchy, springy
ability, it requires extra effort to breathe; this results in a person feeling short of breath. Airway
obstruction occurs in emphysema because the alveoli that normally support the airways open cannot do
so during inhalation or exhalation. Without their support, the breathing tubes collapse, causing
obstruction to the flow of air.

Symptoms of COPD
COPD is identified usually with symptoms including:
• Shortness of breath, especially with physical activity.
• An uncontrollable cough.
• Excessive mucus production.
• Wheezing.
• Chest tightness.
Diagnosis of COPD
Symptomatic patients are evaluated with a breathing test that measures airway obstruction called
spirometry.
In the majority of COPD cases, patients remain undiagnosed or wrongly diagnosed in clinical practice
because of several reasons:
• There is a myth that COPD is mainly a condition that predominately affects tobacco smokers,
this leads to the under-diagnosis of COPD in non-smokers who actually contribute almost half of
all COPD cases.
• Lack of utilizing spirometry as a diagnostic tool, because of a knowledge gap for physicians and
the non-availability of spirometers.
• There is difficulty in differentiating COPD from asthma. This confusion leads to under-diagnosis;
delays in treatment allows the state of the disease to progressively get worse.
Treatment
Upon diagnosis, physicians suggest that patients who smoke should cease smoking before and during
treatment. Medications are typically prescribed to widen the airways (bronchodilators), reduce swelling
in the airways (anti-inflammatory drugs, such as steroids), and/or treat infection (antibiotics). COPD can
also cause the oxygen level in the blood to drop below healthy levels; if this occurs, supplemental
oxygen will be prescribed. As a process of these prescribed medications and for effective drug delivery,
assisted medical delivery or medical devices are employed.
Respiratory medical devices play a significant role in the management and treatment of COPD.
Epidemiology of COPD
Mortality
In a global burden study performed in 2015, it was found that 3.2 million people died from COPD
worldwide, an increase of 11.6% when compared with 1990. COPD is an important contributor to the
burden of non-communicable diseases. COPD kills more people than HIV-AIDS, malaria and tuberculosis
combined in the South-East Asian region. Although much of the burden is either preventable or
treatable with affordable interventions, this disease has received less attention than other non-
communicable diseases.
Prevalence
The burden of COPD across the world is high. In 2015, the Centre for Global Health Research and the
World Health Organization (WHO) performed a systematic review to understand the COPD prevalence
trends across the globe. To understand the disease progression levels and growth rates, the outcomes
were compared between two years: 1990 and 2010.
© 2018 | BCC Research LLC HLC167C Anesthesia and Respiratory Devices: Global Markets | 28
The highest COPD prevalence estimated in this study was observed in the American region (as defined
by the WHO), with an estimated prevalence of 13.3% and 15.2% in 1990 and 2010, respectively. In the
WHO’s Western Pacific region, the number of COPD cases reported in 1990, was approximately 60
million and in 2010, almost 113 million cases were reported with COPD.

Inhalation route is the fastest and most effective method of delivering medications to the respiratory
system during treatment of COPD. As a part of treatment and management of COPD, inhalers and
nebulizers play the most dynamic role.
Usage of medical devices is not just restricted to treatment of COPD although they are necessary for
effective diagnosis, drug delivery, critical care management and respiratory support for individuals with
COPD. Because it is a chronic disease, COPD creates an abundant demand for the usage of respiratory
devices like Inhalers, nebulizers, spirometers, oximeters, breath filters and respiratory disposables.
The prevalence of COPD has never declined and is expected to continue the same pace of growth across
the globe. Unlike other diseases, COPD’s rise is dependent on urbanization, as most of the studies
proved that urban people are severely prone to COPD. In the respiratory devices segment, COPD has an
ultimate share and in most markets the percentage of shares is almost dependent on the COPD
prevalence rates. Hence, COPD is considered a key driving factor for the anesthesia and respiratory
devices market.

High Prevalence of Obstructive Sleep Apnea (OSA)
Obstructive sleep apnea is caused by a blockage of the airway, usually when the tongue collapses
against the soft palate and the soft palate collapses against the back of the throat during sleep, and the
airway is closed. This airway obstruction creates a real-time obstruction to the respiratory process that
leads to the disturbance of sleep and a low quality of sleep. The effects of OSA are not just restricted to
bad sleep quality but also to respiratory health, causing deoxygenation. Patients have a high risk of
developing cardiovascular disorders and chronic disorders like diabetes and obesity. Risk factors for OSA
include obesity, aging, gender, menopause and ethnicity.
Types of OSA
In central sleep apnea, the airway is not blocked but the brain fails to signal the muscles to breathe.
Complex sleep apnea, as the name implies, is a combination of the two conditions. With each apnea
event, the brain rouses the sleeper, usually only partially, to signal breathing to resume. In those
individuals with severe sleep apnea this can happen hundreds of times a night, often most intensely late
in the sleep cycle during rapid-eye-movement (REM) sleep. As a result, the patient’s sleep is extremely
fragmented and of poor quality. Meanwhile the disorder continuously reduces the oxygenation of the
blood, further stressing the sleeper’s physical system.
Symptoms
OSA is generally observed with frequent episodes of any of the below and sometimes multiple
symptoms together on a frequent basis:
Diagnosis
The only accurate diagnostic tool for determining the presence of OSA is a sleep study via
polysomnogram. This is conducted by a certified sleep specialist during a patient’s through-the-night
visit to a sleep laboratory that will include monitoring, among other things, blood oxygen levels,
respiration rate, brain-wave activity, leg movements, and most significantly, the apnea-hypopnea index,
which is the number of apnea and hypopnea (partial inhalation) incidents the sleeper experiences per
hour. Home based testing, known as limited channel testing (LCT), is also available for certain patients. A
patient’s utility of LCT must be evaluated by a clinician to verify eligibility.

Prevalence of OSA
According to the American Sleep Association’s sleep disorder statistics, it is estimated that 25 Million
adults in the United States (U.S.) have OSA. The prevalence of OSA has been strikingly high and is
estimated to be 27.5% in male and 15% in female population. This creates an alarming situation.
In 2008, a population-based epidemiological study was performed to determine the prevalence of OSA;
the study found that the global prevalence of OSA is 21.7% for adult men and 13.2% for adult women in
the general population.
The prevalence of the disease was found to be higher in specific population subsets, including
overweight or obese people, diabetic and hypo or hyperthyroid patients. The study depicted the fact
that OSA prevalence estimates across the globe including North America, Europe, Australia, and Asia are
not substantially different suggests that this disease is common; OSA is not restricted only to developed
countries but is also common in developing countries.
Table 2
Prevalence of OSA
For treatment and disease management, various respiratory devices are utilized like respiratory masks
including nasal pillows, nasal masks, full face masks and pediatric masks, as well as respiratory devices
including positive airway pressure devices (CPAP, Bilevel or BPAP and APAP), oxygen concentrators,
humidifiers and ventilators.
© 2018 | BCC Research LLC HLC167C Anesthesia and Respiratory Devices: Global Markets | 32
As there are no prevention techniques available to eradicate or arrest OSA, this area is expected to be a
boon for the respiratory device business and hence, the majority of the medical device manufacturers
are investing in the development of novel and advanced OSA devices.
Air pollution occurs both in both indoor (e.g. household) and outdoor environments. However, in terms
of risk index, outdoor pollution is more severe. Below are a few emissions which are considered serious
pollutants:
The above pollutants are considered to have a range of undesirable impacts, including damage to
human health, ecosystems, food crops and built environments. An evidence-based attribution of the air
pollutants to specific health or environmental outcomes is complex as some pollutants act as pre-
cursors to others. For instance, SO2 and NOx will not have serious effects directly but when together
emitted or mixed together, these chemicals have a tendency of reacting in the atmosphere to form
strong particulate matter compounds. Such particulate matter compounds cause significant damage to
the environment. While the sources of each pollutant differ, most are results of fuel combustion and
uncontrolled or faulty industrial activities; pollutants are released as by-products of these processes.
© 2018 | BCC Research LLC HLC167C Anesthesia and Respiratory Devices: Global Markets | 33
In 2011, the WHO concluded that exposure to indoor air pollution doubles the risk of COPD, pneumonia
and other acute lower respiratory infections, and may account for roughly 800,000 annual worldwide
deaths in children five years of age and younger. In various studies performed across the globe, it was
statistically found that air pollutants or harmful emissions tend to lead to acute lower respiratory
infections in children and COPD in adults at two to three times the rate when compared to
unpolluted/less polluted regions. Most importantly, with the extensive industrialization and a lack
government and pollution board guidelines, Asia has emerged as a hub for pollution and offers the
highest risk for chronic respiratory disorders.
The need for surgeries has been growing across the globe irrespective of a country’s economy, health
conditions or healthcare spends. In 2016 the WHO performed an analysis to understand the surgery
levels at a global level. In this study, the WHO estimated 226.4 million operations for the year 2004. For
the year 2012, the WHO estimated the total global volume to be 312.9 million operations. An increase of
38.2% in the span of seven years is significantly high. In 2012, the estimated mean global surgical rate
was 4469 operations per 100,000 people per year.

Market Restraints
Regulatory Constraints
The medical device industry throughout the world has become very competitive. To handle the
competition efficiently and to gain a better market share, most manufacturers aim at new designs and
innovative devices to be introduced into the market with enhanced performance. Every product that is
planned to be launched in a specific region must face mandatory regulatory hurdles. To ensure patient
safety, various regulatory bodies introduce regulations to control the medical devices entry at a global
or country level.
Regulatory bodies are mainly responsible for prequalifying medical devices intended for human use.
Among the global regulatory firms, the U.S. Food and Drug Administration’s (USFDA) Center for Devices
and Radiological Health (CDRH) plays a key role in regulating firms who manufacture, repackage, relabel,
and/or import medical devices sold in the U.S. Being the world’s biggest market, the U.S. market is vital
for any device’s success and the devices that gain the USFDA approval have the best chances to cross
other countries’ regulatory pathways.
Medical devices are classified into Class I, II, and III. Regulatory control increases from class I to class III.
The device classification regulation defines the regulatory requirements for a general device type. Most
class I devices are exempt from Premarket Notification 510(k); most class II devices require Premarket
Notification 510(k); and most class III devices require Premarket Approval.
The anesthesia and respiratory devices are mostly classified under Class II as per regulatory classification
rules.
Governments in both in the U.S. and Europe have implemented strict regulations for the production,
standardization, and utilization of anesthesia and respiratory related devices. In Europe, the European
commission is the regulatory authority that regulates the Medical Device Directories (MDD), this agency
monitors the usage and approval of medical devices in the European Union (EU). In the U.S., it is
mandatory for the manufacturers and initial distributors of medical devices to register their
establishments with the USFDA and list their products and follow the standards set by the authority.
Communaute Europeenne (CE) marking certification is mandatory for all the medical devices sold in the
EU. This certification requires only proof of safety, whereas the USFDA requires evidence of both safety
and efficacy of a device.
Impact on Market
As discussed above, regulatory bodies control the entry of novel products which are scheduled to enter
the market to ensure patient safety. As a process of regulatory pathway, many manufacturers are still
fumbling at some point of time because of a lack of any individual requirement listed by the regulatory
agencies. Typically, a timeline required for the regulatory approval of medical device post clinical testing
is between three and five years. Despite the abundant care taken by the medical device manufacturers
to follow the regulatory pathway, some devices are rejected or are kept on hold.
Listed in Table 3 below are some of the respiratory devices which were recalled or rejected by the
USFDA in recent years:

Macroeconomic Factors
The world economy suffered in the early months of 2016 and recovered as the year continued. Both
industrialized/developed countries and developing and emerging countries contributed to the upturn.
The total economic growth reported was 3.1%, a slight decrease compared to the previous year at 3.2%.
This growth was reinforced by stable commodity prices and recovering growth rates in China. The surge
in the key interest rate in the U.S at the end of 2015 had only a marginal effect on the global economy.
The Gulf States decided at the end of 2016 to reduce the production of oil, causing a slight price
increase. The year 2016 predominantly featured governmental elections in the U.S., Great Britain and
Italy, which drastically altered the political environment. The consequences for the global economy
cannot yet be determined.
Investment activity in the eurozone augmented cautiously because of the political uncertainty brought
about by the Brexit referendum. However, the British pound fell sharply against the euro. While the
Spanish economy is currently recovering from its sovereign debt crisis, France and Italy are both
suffering from a marked unwillingness to enact reforms to address structural investment activity in the
eurozone.

Impact on Market
The healthcare industry and especially the medical devices sector is a technology and capital investment
based industry which tends to experience high impact from macroeconomic factors. For the year 2016,
though the medical devices industry overall witnessed a growth curve, speculations in the European
market, changes incurred by the U.S. government’s health policies, a decline in private insurance, an
increase in the geriatric population, and poor revenues reported by the insurance companies across the
globe demonstrate the possibility of poor growth.
Currently, medical device manufacturers are experiencing certain pressures because of factors including
reduced health care budgets in many countries, stagnating insurance provider revenues, declining tax
revenues and increased import prices over the preceding two years.
Europe
Western European countries needed to save on healthcare because of a slight drop in economic growth
and obstinate economic pressure. This situation has caused health care spending to only marginally
grow in the EU region countries.
Greece, Italy, Ireland, Portugal and Spain had considerably low budgets that were affected by the
sovereign debt crisis.
Healthcare in Great Britain is facing a huge challenge as the demand for healthcare services has
increased while the budget for medical services has almost decreased.
The Russian healthcare industry was significantly impacted by Russia’s international dealings and the
subsequent economic challenges.
North America
Healthcare spending in North America increased in 2016.
The increase of insurance coverage because of health care reform (Affordable Care Act) in the U.S. led to
an increase in the North Americas’ healthcare spending. It is still uncertain if the trend will continue
following changes in healthcare reform by the new government administration is uncertain.
Japan
After the U.S., Japan is considered as a strategic center and offers significant opportunity for business by
healthcare companies. Japan is known for its growing economy, technological advancements,
encouraging healthcare policies and high pricing opportunity. This scenario, however, has shifted as the
Japanese healthcare system faces progressively more pressures.
The Japanese healthcare system was established in the 1960s and was based on stable demographic
developments; however, the current state is in total contrast as the funding agencies can no longer
sustain the increased spending that is the result of an aging population, increases in chronic diseases
and the emergence of expensive, innovative treatment options. This situation has been worrying the
government and the medical device manufacturers. Recently the Japanese government made plans to
restructure its healthcare system with better funding options to alleviate the funding situation.

Respiratory devices are the highest selling segment in the anesthesia and respiratory devices market.
This segment held a market share of 37.2% in 2016 and is expected to increase its share reaching 37.4%
by 2022. The key aspects of growth rate of these devices are increases in COPD incidence, air pollution
levels and insurance coverage. Although there are many factors that enable the growth of this segment,
its market share is expected decline because the market is established and saturated. Additionally, it is
expected that the average selling prices will decline sharply because of competitive pricing pressures.

Key Companies
Globally, Philips Respironics, Inc., ResMed Inc. and Medtronic plc, are the leading companies in the
respiratory devices market with a combined share of 50.0%. All other international companies compete
for the market shares and in case of Asia-Pacific and South American countries, the local manufacturers
compete aggressively with the leading players.
• Capnographs.
• Spirometers.
• Pulse oximeter systems.
The respiratory measurement devices segment has second highest market share in the anesthesia and
respiratory devices market. It held a market share of 27% for 2016 and is expected to decline its share
reaching 26.3% by the year 2022. The key aspects of growth rate of these devices are increasing COPD
incidence, an increase in the number of hospitals and healthcare facilities in Asia-Pacific countries and
insurance coverage. Although there are many factors that enable the growth rate of this segment, its
market share is expected decline because the market is established and saturated; local manufacturers
will continue to grow and reduce the average selling prices because of competitive pricing pressures.

Key Companies
Medtronic plc, Philips Respironics, Inc. and Masimo Corporation are the leading companies in the
Respiratory Measurement Devices market globally with approximate combined share of 50%. All other
international companies compete for the market shares and in the case of Asian countries, especially
India and China, local manufacturers compete aggressively with the leading players.
Respiratory Disposables
Respiratory disposables are the devices or aids which are used as a part of providing artificial respiratory
support to a patient. These may be employed to assist bigger devices, which are mostly one-time-use
devices for patient safety. There are various types of respiratory disposables used in healthcare; these
types have been considered as a part of this market for this study:
• Patient interfaces.
• Positive end expiratory pressure (PEEP) valves.
• Oxygen masks.
• Resuscitators.
• Aerosol masks.
• Nasal cannula.
• Cardio-pulmonary resuscitation (CPR) barriers.
Respiratory disposables contribute the third highest market share in the anesthesia and respiratory
devices market. This segment held a market share of 12.6% for 2016 and is expected to decline its share
reaching 11.4% by 2022. Although there are many positive aspects like increasing chronic disorders,
ease in availability and insurance coverage, this segment is expected to decline its market share. A few
factors that account to the decline of this segment are a saturated market, high competition from local
manufacturers, reductions in the average selling prices because of competitive pricing pressures, lack of
regulations for disposable products and no abundant patent protected products available.

Anesthesia Machines
Anesthesia machines are the advanced machines that are used to deliver the measured quantity of
anesthetic gases and vapors to the patient safely and effectively. These machines are generally used
along with respiratory devices during surgical procedures. Anesthesia machines are classified into three
types:
• Anesthesia workstations.
• Stand alone anesthesia delivery machines.
• Portable anesthesia delivery machines.
Anesthesia is a common requirement for most of the surgeries. Despite of ample scope for usage,
anesthesia machines currently have a minor market share of 6.3% out of the anesthesia and respiratory
devices market. This segment is expected to grow at rate of 6.4%. A few factors enabling the growth of
this segment are technical advancements in the hospital and healthcare systems of Asian-Pacific and
South American countries, a growing demand for automation and technical sophistications in hospitals,
and increasing skill sets of anesthesiologists.

Growth in the anesthesia and respiratory devises market in the U.S. is the result of several distinct
drivers.
In 2016, healthcare companies found relief from a special tax on medical devices (Medical Device Excise
Tax). The U.S. Congress suspended the tax for two years in 2015.
A slight dip in the country’s GDP was backed up with a strong U.S. dollar performance and high volatility
in the financial markets.
There was an increase in healthcare spending because of the expansion of insurance coverage arising
from health care reform with the Affordable Care Act.
Growing prevalence of OSA and COPD promises better business for the companies.

Market Scenario
Canada managed to deliver constant growth rates despite the country’s GDP being minorly affected by
the dipping U.S. economy.
The country experienced certain demographic stress as it recovered from damage caused by isolated
incidents, such as the wildfire in Alberta.
As a part of country’s strategy towards firm growth, in October 2016 Canada and the EU concluded the
CETA (Comprehensive Economic and Trade Agreement), which promises new export opportunities for
both sides.
With increasing incidence of OSA and chronic respiratory conditions and sufficient insurance coverage,
the medical device manufacturers can explore better opportunities in the future.

Healthcare spending in Asia-Pacific experienced a visible increase in 2016 because of an expansion of
state healthcare programs and the growing affluence of the population by the key member countries.
India saw an increase in spending because of the government’s resolution to improve healthcare
facilities and upgrade technology.
Healthcare in China continued to grow swiftly. In addition to government initiatives, this growth was
primarily to the result of the increasing affluence of the population and rapidly progressing economy.
Most importantly, the Chinese market has high growth opportunity because of the country’s changing
mindset towards traditional medicine. Both the government and the people of China are shifting their
preference towards modern healthcare.
Japanese healthcare is currently experiencing certain pressures that include an ageing population,
increased prevalence of chronic disorders resulting in a huge flow of claims to the private insurers and
financial pressure reported by the funding agencies in recent past. However, this increased spending is
expected to be dealt with by the government soon; the Japanese government is working towards
revamping healthcare policies for better healthcare support.

Market Scenario
• The healthcare market suffered a certain level of pressure in the Eurozone due to the economic
fluctuations that happened in the member states.
• A few constraining events including the decline of the British pound against the euro that
showed an impact on the U. K.’s market.
• A few member states of the EU faced certain pressures including Spain’s sovereign debt crisis
and both France and Italy faced market unwillingness (rejection of the referendum in Italy).
• The majority of the EU member states reduced or froze the healthcare costs for the cost
optimization. However, all the countries are recovering from the unexpected economic shifts
and are focused to move towards providing better healthcare.
• Despite the discouraging factors, the overall medical device industry managed to grow by 3-4%
which is a positive sign to investors and manufacturers.

Rest of the World (ROW)
The regions covered under ROW here are South America and the Middle East and Africa. With the
altering global socio-economic conditions, these regions have turned out to be the second fastest
growing market.
Market Scenario
South America
The healthcare markets in Latin America grew in 2016 in spite of budget cuts in many countries.
Governments are working to improve their public healthcare systems. In 2016 numerous hospitals were
under construction in Peru and Chile. Brazil kept decreasing its healthcare spending because of difficult
failing consumer trust, economic conditions, and a restrictive monetary policy inculcated by the
government. Despite discouraging economies, Latin America has witnessed highest growth after Asia-
Pacific in the anesthesia and respiratory market.
Low establishment costs, ease in doing business, growing population, increasing industrialization
activities, high prevalence of chronic respiratory infections are few factors that attract the medical
device manufacturers to explore the business in this region.
The import quota in many countries is 80 to 90 percent. Following the settlement of the nuclear dispute
between the U.S., the EU and Iran at the start of 2016, there are a handful of new export opportunities
to the Middle East for medical device manufacturers.

Chapter 7: Patent Review/New
Developments
Significance of Patents
A patent can be defined as "a set of exclusive rights granted by a government office to an inventor or his
assignee for a fixed period of time in exchange for a disclosure of an invention." Applications for patents
are usually handled by a government agency. In the U.S., the U.S. Patent and Trademark Office (USPTO)
issues patent applications, and patents and in Europe, the European Patent Office (EPO) provides the
same service. In the same way, every country which is involved in certain research and development
activities hold a country level patent office.
Table 59
International Patent Offices
© 2018 | BCC Research LLC HLC167C Anesthesia and Respiratory Devices: Global Markets | 110
By granting the right to produce a new product without the threat of competition, patents provide
incentive for companies or individuals to continue developing innovative new products or services. As
pharmaceutical companies spend large sums on research and development, patents are essential for the
pharmaceutical companies to earn a profit.
Patents enable them to recoup development costs and give them time to earn a fair return on their
investment. Effective patent protection provides incentives to research and development and is a key
requirement for raising venture capital. It fosters the technical innovation so crucial to competitiveness
and overall economic growth. Patent rights create an incentive for companies to develop workarounds
to patented inventions, thereby creating improved or alternative technologies that might not otherwise
be developed.
Patent Analysis
This report presents the number of patents issued to some of the key players in this market in 2016 and
2017. This demonstrates the strategies of a company towards gaining more market share by relying on
innovation and product development to compete with new products scheduled to come to market.
• In 2016 patent and trademark offices around the world issued 234 new patents to Draegerwerk.
• In 2016 the company applied for another 86 patents at international patent offices.
• In 2015 patent and trademark offices around the world issued 147 new patents to Draegerwerk.
• In 2015 the company applied for another 93 patents at international patent offices.

Loss of Exclusivity
The loss of exclusivity of the patents is very crucial as the competitors would have abundant opportunity
to explore the markets vigorously. ResMed consists of multiple patents to be expired in the coming
years. Of the company’s patents, 222 United States patents and 483 foreign patents are due to expire in
the next five years. Below is a year-wise projection of loss of exclusivity of the company’s patent

Chapter 8: Pipeline Analysis
Definition of Pipeline
A pipeline product is a series of products, either in a state of development, preparation, or production,
developed and sold by a company, and ideally in different stages of the product’s life cycle.
A concept of having a product as a part of the pipeline usually begins with brainstorming by the
companies to come up with new ideas for possible products that a firm can add to its product portfolio
that meets unmet demand in a market and gain or withstand the market share.
Pipeline Analysis
In the patent analysis section, this report attempts to depict the key player’s research interests and
pipeline product listing. This following report identifies products in the pipelines of various companies
leading this market.

Table 67
Pipeline products of Key Players
In a highly competitive business like the medical devices sector, the need for innovative products in any
market segment is required. It was observed that the pipeline products are low in number at this period
but with the strong patent portfolio of the companies, it is expected that the companies are being
futuristic and will gain a robust pipeline to maintain or increase their market share.
It is anticipated that all the products currently in the early stages of development by the key players will
progress drastically towards clinical development and regulatory pathways when compared to previous
generations. This is because standardized healthcare systems are emerging in markets and there are
reduced local approval challenges.

allow them to provide better access to healthcare, improve the affordability of care, and improve the
quality of patient outcomes.
Technology upgrades and innovative solutions are employed by the manufacturers to provide products
that meet these customer requirements while maintaining cost effectiveness. Every product that is
being sold or that is scheduled to enter the market is subject to regulation by numerous government
agencies, including the USFDA, as well as various laws and regulations that apply to claims submitted
under Medicare, Medicaid or other government funded healthcare programs.
Controlling Factors
The medical device industry grew at a growth rate of 4%. It was observed that manufacturers especially
in the Anesthesia and Respiratory devices business segment experienced decreases in revenues or
minimal growth. A few factors that controlled this market are mentioned below:
• The effects of inflation and the impact of a stronger U.S. dollar compared to foreign currencies.
• Lower prices because of competitor pricing pressure.
• Decreased research and development budgets reported by many companies because of a hike
in the cost of production.
• Increased labor costs and need for automation of industrial technology upgrades which incurred
increased operational costs.
• Stagnant national healthcare budgets in most of the EU and in developing countries in recent
years have created pressure on prices in most of the products.
• Market conditions are experiencing dynamic shifts and becoming a harder environment for the
manufacturers, and the competition for market share is intense.
• An emphasis by regulatory agencies on stricter standards is being imposed on suppliers.
• Regulatory constraints have been continuously affecting the market and especially the local
registration of novel products remains a challenge, often taking years.
• Few companies, especially the leading companies, are still suffering with complex sales and
contracting models. This is based on aged sales strategies and an inability to reach the dynamic
mindsets of start-up or fast progressing companies.
• Increased import and export costs and taxes by many governments across the globe.

Market Opportunities
The anesthesia and respiratory devices industry observed a sluggish growth from 2016 to 2017.
Numerous controlling factors listed above have accounted to the decline of the growth rate. However,
the positive side is that the sector is a well-established market and multiple probing drivers offer
multiple opportunities.
The majority of the key players including Philips Respironics, ResMed, Medtronic, Ambu A/S and Fisher
and Paykel Healthcare have adopted strategies to invest significant resources on the development of
new product ranges that are highly efficient and cost-saving. This can be proven by the increase in the
research and development budgets of most of the companies; additionally the companies are
aggressively working towards reducing costs of production through employing the latest cost-effective
techniques. The rise in the number of patents filed or consistent patent filing or issuance patterns of the
manufacturers clearly demonstrate the vision towards product innovations. Several countries have
implemented encouraging strategies as a process of uplifting the local companies or research institutes
to dominate or aid market growth through their innovations.
• Reduced cost of production when compared to other zones in terms of establishment costs and
availability of skilled labor.
• Cost effective sales and marketing capabilities because of the low-cost of workforce and
operations compared to western counterparts.
• Encouraging government policies, like the “Make in India” Initiative of the government of India
to welcome manufacturers and provide cost-effective production units with numerous
subsidized benefits.
• A change in the stern regulatory pathways to move products to market especially China and
India.
© 2018 | BCC Research LLC HLC167C Anesthesia and Respiratory Devices: Global Markets | 128
• Increase in the country level GDPs and healthcare spends.
• Increase in reimbursement and insurance coverage.
• Promising socioeconomic development and political scenario.
• Increasing demand for single-use devices in all regions. This is because of low production costs
incurred, increased demand, less opportunities of contamination and increased patient safety;
most of the companies are exploring options to design, deliver and market cost-efficient single-
use devices.

LMA Airways: There are multiple devices which are produced under this product line namely LMA
laryngeal masks- Used by anesthesiologists and emergency responders to establish an airway to channel
anesthesia gas or oxygen to a patient's lungs during surgery or trauma.
The LMA Protector Airway- The company’s latest airway management device, is the single-use laryngeal
mask with a dual gastric drainage channel and pharyngeal chamber designed specifically to channel high
volume, high pressure gastric contents away from the airway. It also integrates company’s Second Seal
technology to isolate the respiratory tract from the digestive tract, reducing the risk of aspiration of
gastric contents. The LMA Protector Airway also includes Teleflex’s Cuff Pilot technology, which enables
clinicians to confirm that the inserted cuff is properly inflated and to monitor pressure levels.
