MRI OF THE LEFT KNEE JOINT


MR imaging was performed on an advanced 3.0 Tesla, 32 channel digital broad-band MR system using a dedicated multi-
channel phased-array surface coil with axial, coronal and sagittal FSPD images and coronal T1-weighted images.

Clinical profile: Osteoarthritis.


Diffuse maceration with poor visualization of the body and posterior horn medial meniscus with 5.8mm medial meniscal
extrusion. The anterior horn, body and posterior horn of the lateral meniscus including the meniscal root ligaments,
appear normal in morphology without obvious tear.
There are moderate degenerative changes in the femorotibial compartment in the form of marginal osteophytes, tibial
spiking and chondral denudation in the medial femorotibial compartment and chondral thinning in the medial femorotibial
compartment. Subcortical cystic area is seen in the tibial intercondylar eminence. The rest of the distal femur and
proximal tibia appear normal in alignment and marrow signal with intact overlying articular cartilage. The patella shows
Wiberg’s type II morphology with a long patellar nose, normal craniocaudal & transverse position and marrow signal with
marginal and patellar polar osteophytes. There is a medial patellar plica extending over the medial trochlear articular
surface. Superficial chondral fibrillation is seen over the medial patellar facet and trochlear groove. The rest of the
articular cartilage lining the patellar and trochlear facets appear normal in thickness and morphology. The quadriceps
tendon & ligamentum patellae appear normal.
The anteromedial and posterolateral bands of the anterior cruciate ligament appear intact and there is increased signal
within the ACL. The posterior cruciate ligament is intact. The medial collateral ligament is intact. The iliotibial tract, fibular
collateral ligament and the tendon of the biceps femoris are intact. The popliteus tendon appears normal.
Mild-to-moderate synovial thickening is seen in the suprapatellar pouch, medial & lateral patellofemoral recesses. Loose
bodies are seen in the suprapatellar pouch measuring 8.3 x 5.1mm & 6.7 x 6.8mm. Multiple loose bodies are also seen,
the largest measuring 6.8 x 6.9mm. Two large loose bodies are seen in the posterior aspect of the inferior medial
femoral condyle, larger measuring 5.0 x 15.2 x 15.0mm.
OPINION: MR scan findings are suggestive of :-
· Diffuse maceration with poor visualization of the body & posterior horn medial meniscus with 5.8mm
medial meniscal extrusion.
· Chondral denudation in the medial femorotibial compartment and chondral thinning in the medial
femorotibial compartment; subcortical cystic area in the tibial intercondylar eminence.
· Medial patellar plica extending over the medial trochlear articular surface; superficial chondral
fibrillation over the medial patellar facet and trochlear groove.
· Multiple loose bodies, as described above.
