______________________________, do hereby appoint, name and constitute
________________________, both of legal age, Filipinos, and residents of
_______________________________, to be my true and lawful ATTORNEY/S-IN-FACT,
for me and in my name, place and stead, to do and perform any of the following acts
and things to wit:

1. To secure, fill out, and prepare documents to be submitted to the relevant

government agencies, including, but not limited to, the Bureau of Internal
Revenue, City Assessor’s Office, and the Registry of Deeds, in relation to the
payment of taxes, fees, and charges, if any, assessed against the real property
located in San Pablo City, Laguna covered by Transfer Certificate of Title No.,
which is the subject of the;

2. To transact with the employees of the relevant government agencies,

including, but not limited to, the Bureau of Internal Revenue, City Assessor’s
Office, and the Registry of Deeds in relation to the securing of documents and
payment of taxes, fees, and charges, if any, assessed against the real property
located in San Pablo City, Laguna covered by Transfer Certificate of Title No.,
and eventually the transfer thereof in my name;

3. To process with and secure the new Transfer Certificate of Title from the
relevant government agencies, including, but not limited to, the Bureau of
Internal Revenue, City Assessor’s Office, and the Registry of Deeds, the
documents in relation to the transfer of real property situated in San Pablo
City, Laguna covered by Transfer Certificate of Title No. in my name after the
payment of the necessary taxes, fees, and charges, if any;

4. To pay the required taxes, charges, and other fees before the Bureau of
Internal Revenue, City Assessor’s Office, and the Registry of Deeds in relation
to the transfer of the real property located in San Pablo City, Laguna covered
by Transfer Certificate of Title No. in my name;

5. To be my legal representative before the relevant government agencies

including, but not limited to, the Bureau of Internal Revenue, City Assessor’s
Office, and the Registry of Deeds in relation to the transfer of the real
property situated in San Pablo City, Laguna covered by Transfer Certificate of
Title No. in my name.

HEREBY GIVING AND GRANTING unto said ATTORNEY/S-IN-FACT full power

and authority to do and perform every act whatsoever requisite and necessary to be
done in and about the premises, as fully to all intents and purposes as I might or could
lawfully do if personally present, hereby ratifying and confirming all that said attorney-
in-fact shall lawfully do or cause to be done by virtue of these presents.
