As a relative of the deceased, I am the one in charge of the post-mortem affairs of my uncle;

I am executing this affidavit to attest to the truthfulness of the foregoing to be able to perform
the following:
a. secure the death certificate of my uncle Jose Jaucian Tapan Jr. from the Philippine
Statistics Authority,
b. recover the remains of my brother from ______________;
c. to process and transact with any and all offices or entities for the post-mortem activities
of my uncle.
