AFFIDAVIT OF UNDERTAKING

I, Merlita D. Alsisto, Filipino citizen, of legal age, with address at #42 Upper
Market Subdivision, Baguio City, after having duly sworn to in accordance with law,
do hereby depose and state that:

1. I am the registered leaseholder of Stall No. 245, Block III Section, with
approved additional line of business of retail sale of dry goods pursuant
to BCMA Res. No. 008-2019;

2. I have no any other existing stall;

3. I was never awarded any stall by the Baguio City Market Division;

4. I execute this Affidavit of Undertaking to attest to the truth of the

foregoing and for all legal intents and purposes this may serve.

IN WITNESS WHEREOF, I have hereunto set my hand this 10th day of March
