CONSENT FORM

Dear Respondent, 
 
We would like to ask for your permission to conduct a survey for our research. Our
research aims to determine the level of awareness of students of Ateneo de Naga University in
terms of cryptocurrency. The study also seeks an explanation as to how profile and exposure to
cryptocurrency affect the level of awareness of the students. If you decide to willingly participate
in this study, please note that your participation is voluntary and there would be no any risks.
Any information that is collected will be kept confidential and will never be disclosed to anyone.
Rest assured that your responses will be used for academic purposes only. Answering the
questions as best as you can is much appreciated. This survey would take approximately 10
minutes of your time and will be conducted through google forms. Please don’t forget to submit
