representative after inspection.

My representative understands that she would be required to produce her TIN ID card for
identification and record purpose when collecting the said groceries for me.

I also confirm that my representative shall have the authority to sign for acknowledgment of the
receipt of the said groceries. I understand that I shall be fully responsible for the nondelivery, if any, of
the said groceries from my representative.
