Promissory Note

Greetings! I am Marialle Sam G. Balasta from the municipality of Gamay, N. Samar

but currently studying through online modality at Leyte National High School, Tacloban

City. I hereby promise to get the an authenticated copy of the following requirements

needed; Photocopy of Form 138, Photocopy of certificate of graduation, and photocopy

of Good moral character. Since, it is needed for the school registrar signature, upon the

situation of Tacloban City under GCQ, it is prohibited for children 18 years old and

below to go to Leyte National High school. I guarantee to obtain and pass the

requirement when school has reduce the strict policy.

I am hoping and looking forward for the kind consideration upon this situation. To
