(Hereinafter referred to as Party No. 2) witnesses as follows:
 
1. That Party No. 1 has not married and she does not intend to marry in future.

in adoption on......................... as her son.
 

3. That Party No. 2, after obtaining consent of his wife, has on the aforesaid date given the

customary with the parties, in the presence of relatives and friends of the parties.
