swear on my own volition that I will personally take the ONLINE RESIDENTS’ IN-
SERVICE EXAMINATION (RISE 2021) to be conducted by the POGS Council for
Residents’ Education, Enhancement and Development (CREED) on September 19, 2021
online via the POGS Moodle Learning Management System and further attest to the
following statements:

I further declare and affirm that the answers I will supply in the said examination
are solely my own, derived from my personal knowledge and shall be without
assistance, advise or consultation whatsoever from any third person, printed and
electronic copies of instructional materials, or other online learning resources.

I further acknowledge and willingly accept that any wrongful act performed can and
will be subjected to disciplinary action and that any form or act of academic
dishonesty, cheating or falsification committed therein will correspondingly be
imposed penalties upon the discretion of CREED and approved by the POGS
Board of Trustees. I also understand that failure to execute this HONOR CODE
can be a ground for invalidating the results of my RISE 2021.
