LIST OF REQUIREMENTS
APPLICATION FOR SPECIAL MINERALS EXTRACTION PERMIT
DOCUMENTS REQUIRED

1. Filing Fee (PhP 10.00 per hectare plus PhP 20.00 pursuant to Presidential
Decree (P.D.) No. 1856: but not less than PhP 5,000.00

2. Duly certified copy of the charter

3. Approved Feasibility Study Report of the Government Project

4. Location Map/Sketch Plan (1:50,000 NAMRIA topographic map) showing

coordinates/boundaries (in tabulated form) with major environmental features/other
projects (prepared, sealed and signed by a deputized Geodetic Engineer)

5. Exploration Work Program (MGB Form No. 5-4) or Development/Utilization/

Operation Work Program (MGB Form No. 6-2), whichever is applicable, duly
prepared, signed sealed by a licensed Geologist in the case of the Exploration
Work Program or a licensed Mining Engineer for the Development/Utilization Work
Program, or the appropriate and authorized technical staff for the Operation Work
Program

6. Environmental Compliance Certificate

7. Environmental Work Program or Environmental Protection and Enhancement

Program (EPEP)

8. Social Development and Management Program

9. Area Status and Clearance

10. NCIP Certification Precondition

11. Proofs of project approval by any two of the following:

a. All Sangguniang Panlalawigan concerned
b. All Sangguniang Bayan concerned
c. All Sangguniang Barangay concerned

12. Supporting documents to the operation
