Re: Ms. Isabel Garcia, aged 20

I am pleased to update you on the progress of one of your patients, Ms. Garcia who has recovered well
in our facility and is ready to be discharged today.

Ms. Garcia was admitted on 23rd May 2015 complaining of one –week history of painful stiff joints along
with sensitivity to light. In addition, she reported to have a headache, stiff neck and skin rash associated
with easy bruisability. On assessment she was afebrile; however, significant abnormalities inform of
neck rigidity and petechial rash over abdomen and legs were found. A provisional diagnosis of meningitis
was established and immediate lumbar punctures along with other pertinent investigations were
ordered. Empirically she was commenced on Inj. Dexamethasone and Inj. ceftriaxone. Subsequently,
laboratory results including CSF analysis had confirmed the diagnosis of Neisseria meningitidis;
therefore, the antibiotic was upgraded to Inj. Benzylpenicillin. Additionally, the Department of Human
Services was notified and an immunization of her family members was re-ensured. With the appropriate
treatment she has been recovering well.

Post –discharge, Ms. Garcia would require a close monitoring and seeking the medical attention in case
she begins to exhibit signs of unexplained illness. Would you mind in arranging chemoprophylaxis for
people who had recently come in close contact to Ms. Garcia.
