AFFIDAVIT OF LOSS

I, FERNANDO JOSE C. SANCHEZ, of legal age, Filipino, single and a resident of Blk 17 Lot 6
Landaska, Torsillo St. Dagat-dagatan Caloocan City, after having been duly sworn to in accordance with law,
depose and state THAT:

1. That as a bona-fide student of ARELLANO UNIVERSITY MALABON-ELISA ESGUERRA

CAMPUS I was issued a Student Identification Card (ID) and Graduated last May 24,
2019 ;

2. Sometime on August 2019, said Student Identification Card (ID) was misplaced and got lost;

3. Efforts were exerted to locate said ID, but in spite of diligent search, it could not be found and the
same is now beyond recovery;

4. As such, I am executing this Affidavit of Loss to attest to the truth of the foregoing and to support my
application for the issuance of a New School ID, in lieu of the one that was lost;
