PREAMBULE
INTRODUCTION

This document helps to install, configure and start using your NYSSTAR system from
Instrumentation Difra.

This manual is the user manual of the NYSSTAR, digital videonystagmography system
manufactured by Instrumentation DIFRA in Belgium. This allows to carry out diagnosis of the
vestibule.

Important note: at every person who will use the NYSSTAR, it is highly recommended to
read this manual and to master this device, before using it on patients.

As with any piece of sophisticated equipment, maximum benefit will be derived if special attention
is paid to the care and maintenance of the device in order to master its capabilities and functions.

INTENDED USE
The NYSSTAR is a medical device manufactured in Belgium by Instrumentation DIFRA. The
NYSSTAR ia digital videonystagmography system. It is designed to allow qualified personnel to
visualize eye reaction of the patient and to track and measure dispalcement of the patient's pupill.

NYSSTAR is a part of an integrated system. It can not be use alone, it has to be used with the
DISOFT software, furnished with NYSSTAR, and a compatible computer. The complete
description of utilization of DISOFT is not made in this manual, please refer to the corresponding
manual for this.

AVAILABLE MODELS
The VIDEOSCOPY videonystagmoscope is now only available in one model, but it will soon be
joined by a second one allowing to connect two cameras simultaneously.

UNPACKING INSTRUCTIONS
EXAMINATION OF THE PACKING
Before opening the package, please inspect the packing against all noticeable damage. In that case,
immediately advise the carrier of the damages and keep the carton as proof.

The NYSSTAR is delivered in a package designed for transportation. Keep it in case of back
shipment.

EXAMINATION OF THE DEVICE


Neither the device nor its accessories must present any default of appearance at its reception. In
case of problems, contact the carrier and your nearest representative of Instrumentation DIFRA.

COMPLAIN IN CASE OF DAMAGES


Any missing part or malfunction should be immediately reported to the supplier of the device.
Attach to your complaint a copy of the invoice, serial number and a detailed report of the problem.
The serial number is on the camera on the identification label.
Instrumentation DIFRA takes no responsibility in case of misuse of the device.

DISOFT INSTALLATION
Do not connect the camera or the NysStar
interface to the computer before being
allowed to.
INTRODUCTION
The software needed to install DISOFT can be provided on a USB stick, a CD or downloaded
through the FTP server from Instrumentation Difra.

The installation process will be described for a new installation in the chapter “New installation”. If
the NYSSTAR is used to update an existing DISOFT installation, please follow the DISOFT update
process described in the user's manual of DISOFT installation. Once the software is updated,
follow the description of the configuration for NYSSTAR.

COMPUTER MINIMAL CONFIGURATION


NYSSTAR is not a stand alone product. It requests a computer and the software DISOFT. The
computer has to satisfy the followinfg characteristics otherwise the system will not work properly.

• Operating system : Windows XP 32 bits, or Windows 7 32 bits;


• Processor : Intel Core 2 Duo (Core Quad recommended for binocular system) or Intel Core
I7 (Core I3 and Core I5 does NOT work);
• Chipset : Intel recommended, (work also with some NVidia chipsets, AMD chipsets don't
work !!!);
• Memory : 3GB RAM;
• Hard disk : 500 GB;
• Graphics card : OpenGL compatible;
• Other : 2 x USB 2.0, 10/100 Mbps RJ45 Port (Fast Ethernet).

NEW INSTALLATION
This is the process to be followed to install the NYSSTAR on a new computer.

Two files are requested for the install: the file “setup.exe” and the folder “ISSetupPrerequisites”. If
one of them is missing the install process will not be possible. Others folders contain drivers and
tools to install and used devices.

The operating system used to explain the software install is Windows 7 PRO 32bits in english. The
software can also be installed on Windows XP PRO 32bits and on other release of Windows 7
32bits. No support will be provided on Windows Vista and the software can not be run on a
Windows 64bits operating system.

Log on the computer as “Administrator” or with an account with full administrator rights. Without
full rights on the computer, some parts of the install process will not be done correctly and can
provoke errors in the utilization of the software even is the install process seemed to be correct.

Double click on the file “setup.exe” to launch the wizard that installs and configures DISOFT.

First of all, the operating system will display a window asking you to confirm the start of the install
process. Accept it or the install process would not work. Then, the following screen will be
displayed.

In this window, the release version of the software that will be installed is indicated. In this
example, the release is 1.23.02.024. Click on “Next” to continue the process.

Please read our end-user license agreement. If you accept it, check the right case and click on
“Next”. If you do not agree with this license, the software can not be installed.

DISOFT is now installed on the computer. The process also installs drivers for the Rhinoceros and
for NYSSTAR. Depending of the complete set-up of the examination room (kinetic stimulator,
visual stimulator, …), others drivers may need to be installed and the the configuration of DISOFT
may also need to be adjusted. For this, please see User's manuals of others devices from
Instrumentation Difra.

CONFIGURATION

In a new installation, the configuration of DISOFT will be done automatically, so this has not to be
done. This chapter will be necessary to configure DISOFT to install a NYSSTAR on an older
installation of DISOFT.

Follow the process describes to reach the configuration part of DISOFT. This part is called “Difra
technical support”. This part of the software allows to adapt DISOFT to each hardware
configuration from customers. Here are explanations specific to the configuration for the
NYSSTAR. Other configurations are not affected by the utilization of the NYSSTAR.

Click on the “Tool button” to start the configuration.

Select the upper screen tab labeled “E.N.G./V.N.G.” and then “V.N.G. (Nysstar)” as second choice.
Check “enabled” in the V.N.G. (Nysstar) screen.

If the torsion is available in the camera, check “Torsionnal”.

With a NYSSTAR II, a system with two cameras, check “Binoccular”.

In the “V.N.G. (Sharc)” tab, verify that “Enabled” is not checked and that the “Frame frequency” is
not zero. Set camera latency to 40ms and set 50 Deg in “Maximum horizontal amplitude” and in
“Maximum vertical amplitude”. Other parameters are not used for the NYSSTAR.

Configurations are finished now, please close the software by clicking on the button “Exit” on the
lower right corner. Then exit DISOFT and check the configuration saveguard in the exit message of
DISOFT.

HARDWARE INSTALLATION
Do not connect the camera or the NysStar
interface to the computer before being
allowed to.
NYSSTAR INTERFACE CONNECTION
The NysStar interface is used to connect cameras to the computer with a locking connection. Please
follow steps and do not connect the camera or the NysStar interface before being allowed to.
1. Turn on the computer and log in as “Administrator” or with an account with full
administrator rights.
2. Take the NysStar interface and set the switch on “O”, the part of the switch with “O” pushed
down. See A on the Illustration 16.
3. Let the switch alone, do not connect camera on it.
4. Connect the power supply on the NysStar interface, see C on the Illustration 16. and connect
it to the mains by plugging it in a power outlet see B on the Illustration 16. The power
outlet should be a 230V AC 50Hz or a 115V AC 60Hz.
5. Connect the NysStar interface to the computer with the USB cable A to B. See D and E on
the Illustration 16.
6. DO NOT CONNECT THE CAMERA.
7. Switch on the Hub by setting the switch on “I”.
8. The computer should recognize the NysStar interface as a generic hub USB.
9. Wait until the computer has finished to install the NysStar interface. The computer will
display a message telling that the new hardware is installed and ready to be used.

CAMERA CONNECTION
Connect a camera on the NYSSTAR interface, see F on the Illustration 16. Even if two cameras are
available, start to connect only one. Depending of the operating software, one of two situations will
take place.
If the computer is running under Windows 7, the camera will be installed automatically and when it
is done, the computer will give a message telling that the camera is ready to be used.
If the computer is running under Windows XP, some steps have to be done. Please follow them as
described here after.

Once all this has been done, the first camera is installed and ready to be used. If a second camera is
available, follow the same process for the second camera without disconnecting the first one.

INSTALLING THE CAMERA UNDER WINDOWS XP

A pop-up window will open automatically.

Now, the camera and the software are completely installed and ready to be use.

FIRST START-UP

If a firewall is installed on the computer, Nysstar will need to be authorized. At first start-up of
DISOFT, when the image of the camera will be displayed, the firewall of the computer will ask you
the authorization for Nysstar. This is normal and without this authorization, the system will not
work. Nysstar uses an internal TCP/IP protocol in the computer to communicate data from camera
to software and without allowing this flow of data, the camera will not be able to send data to
DISOFT.

Here is the example for the intergrated firewall of Windows, if the computer is equipped with
another firewall, refer to its documentation to allow the Nysstar process.

NYSSTAR UTILIZATION
In DISOFT, once the patient is encoded and the exam is started, the image of the eye is displayed in
a new window.

The utilization of NYSSTAR requires most of capacities of the computer. In order to avoid any
problems, we recommend to avoid the use of any other software simultaneously, pay attention to
Windows updates and anti-virus software analyzes. Problems will also be avoided if all sleeping
and stand-by mode are deactivated.

The window of the eye's video will be kept open from exam to exam on the same patient. If you
close the window with the image of the eye, you will have to close the window of the current exam,
the sequence and the window of exam selection to go back up to the patient selection screen prior to
be able to restart the VNG window and make a new exam.

NYSSTAR WINDOW DESCRIPTION


This window is made of three parts. The first part contains the video of the eye, the second part
contains buttons for the Torsion analysis and the third part is made of slide bars for manual settings
of the NysStar. The part of the window containing the manual settings can be displayed or not. The
release version of the NYSSTAR software is indicated on the top of the window, in this example the
release number is the “1.2.0.0”.

The window with the image of the eye can be enlarged. When the cursor change to a double arrow,
left-click and move your mouse to enlarge the window. The maximum size of the eye's video is
from 640x480 pixels at 50Hz.. The position of the window can also be changed Once the position
of the window has been changed, it will be memorized by the software. At next start up, the
window will be opened where it had been closed the last time.

A right click on the image of the eye will display a menu. This menu contains four lines that can be
checked or not.
• Advance: check this line enables to display the manual setting part of the screen;
• Print Stars: enable to display or not the small white cross that show the border of pupil;
• Print Star Center: enable to display the center of the pupil calculated by the NysStar.
• Fixation: not yet available.

The four buttons of the Torsion detection will only be activated if the camera is equipped with this
option. The utilization of the second part of the window will be described in the torsion detection
chapter.

The utilization of the third part of the window will be described in the pupil detection chapter.

PUPIL DETECTION
Once the window with the eye's video displayed, a detection of the pupil has to be made to enable
the eye's tracking.

To detect the pupil:


1. Press down the key “control” (Ctrl) and hold it.
2. Click on the center of the pupil with the mouse.
3. Release the key “control”.

A white cross will then be displayed on the center of the pupil. This white cross will follow
movements of the eye.
Small white crosses are also displayed. They are used to locate the boarder between the pupil and
the iris. If the detection is good, these points are at the boarder between the iris and the pupil, see
on Illustration 24 for a bad detection and Illustration 25 for a good detection.

A bad detection can be detected, if the big white cross does not follow movements of the eye or if
small white dots are not located at the boarder of the pupil and iris. If you have on of these two
situation, do a new detection of the pupil as explain before.

Parameters will be set automatically when you do a pupil detection, they can also be adjusted if
needed. Find a description of these parameters here:
• Luminance: If you have to change one parameter manually, start with this one. To adjust it,
move the cursor in order that the image shows the most details in the iris. When the cursor
is moved from the left to the right side the image will became darker, from white up to
black.
• Pupil color: This is the second parameter to change manually. With this parameter, the color
searched by the system for the pupil will change from gray to black. To adjust it simply
move the cursor to get the more white dots on the border of the pupil.
• Delta Iris Pupil: Adjust the color difference between the pupil and the iris. This is the last
parameter to modify to get most white points on the border of the pupil.
• Radius Min: Minimum radius of the pupil to be detected. Parameter really hard to adjust

better than the automatic process.


• Radius Max: Maximum radius of the pupil, this parameter can be used if an other part of the
screen has almost the same color than the pupil and that some white dots are set close to this
part. Decreasing the value will avoid white dots to be detect too far away of the center of
the pupil.

TORSION DETECTION
The torsion detection is an option on NYSSTAR. The camera needs specific feature to allow the
torsion detection. With the option available, it is possible to activate the torsional in the
DIFRATECHNICAL and in options of each exams.

To activate the torsion for each exam, in DISOFT go in options of exams and check the “Eye
torsion recording” option. The “Averaging” slide bar is a filter from 0% to 100% to smooth the
curve of the torsion. This filter is applied after the recording so it is possible to change it after the
exam and to check its effect on the curve.

Once the exam started and a good pupil tracking set, click on the A button to start the torsion
acquisition, it will display the area of the eye that will be used to detect the torsion.

To increase the precision of the torsion detection, best is to increase the diameter of the area by
using the button B and C and to get a structure from the iris in the area of detection.

The last button D is used to set the reference, to set the zero position of the torsion. When the area
of detection is set and that the patient just look straight without any stimulus, click on this button to
set the zero position.

TROUBLESHOOTING
During the utilization of the NYSSTAR, some problems may occur. Here is a description of what
to do to solve them.

A first point is never disconnecting the camera from the NysStar interface or the NysStar interface
from the computer when DISOFT is started. This will only result in software crashes.

To avoid problems best is not to connect or disconnect any USB hardware (USB key, printer,
mouse, …) when DISOFT and NYSSTAR are running on the computer.

The utilization of NYSSTAR requires most of capacities of the computer. In order to avoid any
problems, we recommend to avoid the use of any other software simultaneously, pay attention to
software automatic updates and anti-virus software analyzes. Problems will also be avoided if all
sleeping and stand-by mode are deactivated.

A last advice is to remove all standby and all energy saving function from Windows.

NO MORE WINDOW WITH THE IMAGE OF THE EYE


The window that displays the eye has disappeared. First check that the window is not minimize to
the taskbar of Windows. If the window can not be found back, please close the exam and start it
again. If at this moment the window is still not displayed, close the exam and the acquisition screen
of DISOFT and start a new one.

One important thing to understand in the behavior of NYSSTAR is that the window of the eye's
video is kept open from exam to exam on the same patient. If you close the window with the image
of the eye, you will have to close the window of the current exam, the sequence and the window of
exam selection to go back up to the patient selection screen prior to be able to restart the NYSSTAR
window and make a new exam.

THE IMAGE OF THE EYE IS FROZEN


Stop the exam and restart it. If this does not solve the problem, close the exam and the acquisition
window. Try to select again the patient in the list and click on “Acquisition”. Start the exam. The
window should be back.

ERROR MESSAGE “CAMERA NOT DETECTED”


If you get the following message, a problem occurs in the USB connection between the camera and
the computer.

"Camera not detected"

This message can appears when the software doesn't find the camera.

Verify that the camera is correcly connected to the computer, that the Nysstar interface is powered
and turned on. Once the trouble resolved, the image of the camera will be automatically displayed.

If the trouble persists, check that the camera is correctly detected by the computer.
