Greetings in St Vincent de Paul!

I am a 3rd year college student taking Bachelor of Science in Accountancy. I am addressing this letter
to you with regards to the document requisition that I have requested last month.

It was on September 2, 2021, when I made a request on the Document Requisition System in the
University’s Live page. I requested two documents to be delivered: Certificate of Good Moral
Character and Certificate of Enrollment. It has been a month and I have not yet received any update
regarding my request. I am really hoping to hear from your office soon regarding this matter because I
will be using these documents for the completion of my scholarship application in our municipality.

I will be expecting to hear from you soon about this. Please let me know if there is any problem with
the processing of the documents that I requested. If you wish to discuss this matter further, kindly
