EMPLOYEE CELL PHONE ISSUANCE AGREEMENT

This document specifies your rights and responsibilities with respect to the mobile phone which shall be
endorsed to you by the company for use in all business transactions, as follows:

1. MAXIDERM maintains the ownership of the phone from the time it is endorsed to you up to the
time of the termination of your employment with MAXIDERM.
2. The phone should be returned to MAXIDERM upon request and termination/separation from
employment.
3. It should be used strictly for business purposes only. Personal calls are not allowed except for
emergencies.
4. Contact information of all MAXIDERM clients and business partners shall be saved in the SIM
installed in the unit.
5. The employee is not allowed to install additional mobile applications on the phone.
6. MAXIDERM reserves the right to inspect the phone and validate cell phone bills
7. You are responsible for protecting the phone from loss, theft or damage.
8. You are responsible for any repair costs or phone replacement costs.
