“Pharmaceutical care is the responsible provision of drug therapy for the
purpose of achieving definite outcomes that improve a patient's quality of life.”

1. Data collection
• interview the patient
• interview is used to systematically collect patient-specific subjective
information
• Record patient's thoughts or feelings and perceptions of his/her condition
or disease.

• Identify medication related problems
