I, GIDEON BDS. BALABA of legal age, Filipino, and resident of Booy,
Tagbilaran City, Province of Bohol, Philippines after having been sworn in
accordance with law, depose and say that:

1.  I am was the owner of a phone with a Smart Talk and Text sim and a cellphone
number 09486102004;

2. Sometime in October 2021, said phone with a Smart Talk and Text sim and
cellphone number was misplaced and got lost in Agusan Del Norte;

3. Efforts were exerted to locate said phone with Smart Talk and Text sim and
cellphone number 09486102004, but in spite of my diligent search, it could not be
found and the same is now beyond recovery;

4. I would like to retain said cellphone number 09486102004, which is the cellphone
number of my lost Smart Talk and Text sim ;

5. Said claim is my right as supported by R.A. 11202 or the Mobile Number

Portability Act of 2018;

6. As such, I am executing this Affidavit in order to cater to any legal purpose it may

serve.
