CANDIDATE CONSENT FORM

This is to certify that you are willing to participate in a Research Study conducted by
Mr.Dileep Babu (@00606510), a Masters level student at University of Salford, Manchester,
United Kingdom.

You will have to attempt an online test (15 mins duration) on Structural Analysis through
Google Forms and submit your answer. The tests are only for understanding your knowledge
of structural engineering. Hence no other personal details will be required.

Eligibility: You should have completed a Degree in Civil Engineering from any recognised
University.
