a.

) Criminal Actions Criminal Actions are those actions instituted arising from Acts or omission
which is punishable by the Revised Penal Code, or by the Special Penal laws.

b.) Yes, The Lawyers from the Public Attorney's Office can only act as lawyers for the accused
in Criminal Cases, because in Criminal Cases, Under the Constitution, It is the right of the accused
that the State shall provide a public defender for the accused, if he cannot afford one. Thus, this is
the duty which the Public Attorney's Office aims to promote, to promote free legal services to
those who cannot afford legal counsel as deemed and provided for by law. Also, in a Criminal
proceeding the Prosecutor shall be the one to prosecute the criminal case against the accused, and
if the complainant-witness, deems it necessary to have other lawyers, a private prosecutor is
available, which is no longer within the services covered for the lawyers from Public Attorney's
Office

c.) Under the 2004 Rules on Notarial Practice, the persons who are authorized to notarized
documents are notary and those commissioning to be notary public, also the judge can notarized
documents. Under the rules on Notarial Practice, to be qualified to be a notary public,

1.) He/ She must be a citizen of the Philippines,

2.) He/ She must be 21 years of age, and

3.) must be a resident of the Philippines for 1 year, also residing in the place of work or
business in which he intends to have his commission be applied.

4.) he must be a member of the Integrated Bar of the Philippines, of good standing,
meaning he must not be convicted of any crimes or offenses involving moral turpitude.

d.) The Judicial Affidavit Rule, it is mechanism adopted by the Court, so as to when a witness/
party is needed to provide or complete his/her testimonies, he can do so in writing, which
materially reduces the material delays brought upon by failure of the witnesses/ parties to attend
during the hearing. It is in accordance with the speedy disposition of cases that is brought upon by
the absence of material key witness or parties.

To ensure the continuity of the proceeding, that even without the actual presence of the
witness/parties, the statements, which is under oath and solemnly sworn by the witness/party,
nonetheless will not put a halt with the proceeding, since the said written manifestations in the
Affidavit shall be deemed as the official and actual statement of the witness/parties. In this Manner.

It also lessens the usage of paper in the Court for instances of the creation of the TSN of
the Court Stenographer. In Cases of Judicial Affidavit, only the material and necessary factual
antecedents in the case is included, unlike in the actual presentation of key witnesses, there could
be additional unnecessary manifestation orally done by the witness.

e.) One of the Objectives of the Revised guidelines for Continuous Trial of Criminal Cases, is to
reduce the possibility of delays in a criminal procedure, by virtue of some tactics or strategies of
lawyers, which is detrimental to the interest of the accused, and contrary to the principle of speed
disposition of trial, (i.e Motion on inhibition, non litiguous motions. )

Another, is to reduce the duration of the Litigation process, which in accordance with the
resolution of the Supreme Court, in accordance with over voluminous clogging of case dockets in
every Court in the country.

INFORMATION

The Undersigned City prosecutor, Ms. May Dulawan, and Mr. Deo Dumpit of the
crime of Reckless Imprudence resulting to homicide, which under Article 365 of the
Revised Penal Code, thereby committed the following acts;

1.) That upon the Order of thereon Department health Secretary, May Dulawan,
and hereby authorize the vaccination of a Vaccine named "Dengvaxia, to all kids from
Five (5) Years old to eleven (11) years old in the vaccination centers located in the City
of Manila.

2.) That, Upon such order, City Health Officer Dr. Deo Dumpit, proceeded,
willfully, feloniously and recklessly, with the vaccination of the One Hundred (100)
children, and that amongst the One Hundred (100), Three (3) Children Died in the
process, including one (1) Loloy.

3) That the said Vaccine was procured by the said accused, with inexcusable lack
of precaution in the acquisition and undue execution of the vaccination process, despite
of them being aware of its low efficacy results and potential risks associated with its use,
which such risk, resulted to the death of three (3) Children.
