AFFIDAVIT

THAT I, WALTER INOCENCIO V. ARREZA, of legal age,

Filipino Citizen with residence and postal address at Brgy. Rosario,
Gumaca, Quezon, after having been duly sworn to in accordance with law,
depose and say:

1. That, I am the Presiding Judge of Municipal Trial Court of

Pitogo, Quezon having been appointed as such on May 21,
2007.

2. That, I was issued a Protocol Plate No. 17D108 for my

previous vehicle and the same was recently transferred to my
current vehicle with Plate No. DAA 2418 since I already
sold my previous vehicle.

3. That, in view of long years of used the said Protocol Plate

17D108 worn out and at present already DILAPIDATED.

4. That, I am executing this affidavit to attest the truthfulness of

the facts stated above, and as a support for the issuance of a
replacement plate for my current vehicle.
