Proof of Academic Identification Form

To purchase any education version of an Adobe product, please complete these steps:
1. Fill out all the fields in this form.
2. Scan or take a digital photo of your valid academic identification*.
3. Email the completed form along with the copy of your ID to AcademicSW@apple.com. Please allow 3-7
days for verification.
4. Once you’ve received a qualification confirmation email from Apple, your order will be processed.

NOTE: Copies of ID cards/documents must be clearly legible and at actual size or larger. Your order must be
shipped and billed to the same address, and shipped to the name of the qualified education user.

Qualification requirements:
You must be a full- or part-time student, faculty member, staff, or employee of a government-certified educational institution in the U.S. or in Canada.

Purchase limits:
You may buy only one copy of any education version of an Adobe product. Education versions of products originally developed by Macromedia (Studio 8,
Dreamweaver, Flash, etc.) are intended for instructional and administrative purposes only and may not be used for any commercial purposes.

*Valid forms of student/faculty identification:

Students
Students must provide a valid, current student ID that includes the following 4 items:
1. Your name
2. The name of your school
3. Validity date(s)
4. A photo
If the student does not have a current student ID with required names, date, and photo, the student must instead provide a valid photo ID, and
corroborating evidence of current enrollment (2008):
• Official current non-photo student ID with name and date
• Official current school transcript or tuition bill indicating name of school and student
• Official current report card indicating name of school and student
• Other official dated proof of enrollment
If the student is under the age of 18 and enrolled at a higher education institution (K-12 students are ineligible) and does not have a valid current Student ID
with photo or credit card, the person purchasing the product for the student must provide a valid current photo ID and corroborating evidence of student's
eligibility:
• Official current non-photo student ID with name and date
• Official current school transcript or tuition bill indicating name of school and student
• Official current report card indicating name of school and student
• Student course schedule or tuition bill with a valid date and printed with the educational entity's logo or seal
• An official letter from the registrar of the educational institution

Teaching Staff
Members of teaching staff must provide the following 2 items:
1. A valid photo ID
2. Corroborating evidence of employment at a qualified institution:
• Official, current institution paycheck stub
• Official letter on school letterhead verifying person's employment

Home Schooling
Those who are home schooling must provide a valid photo ID, and a copy of one of the following:
• Letter of intent to home school addressed to local school district for current school year
• Home School Legal Defense Association membership card
• Home School Charter School membership card
• Book/curriculum receipt for the current year
