It is very important that the petition be in writing and verified. It
must allege the following facts and avernments:

Citizenship Age
The applicant must ba a Citizen of The applicant must be over 21
the Philippines. years of age.

Residency Member of the Bar

The applicant must be a resident
in the Philippines for at least 1
year and maintains a regular
place of work or business in the
City or Province where the
Commission is to be issued.
The applicant must be a member
of the Philippine Bar in good
standing with the clearances from
the office of the Bar Confidant of
the Supreme Court and the IBP.

Conviction Where to file?

Must not be convicted in the first
instance of a crime involving moral
turpitude.
The applicant must decide where
to file the Petition and indicate
the same in the header, taking
into consideration that a notary
public may only perform notarial
acts within the territorial
jurisdiction of the issuing court.
