Dear Sir/Madam:

I am writing this letter to inform you of my resignation as Staff Nurse-Probitionary at Raintree Care
Services & Senior Residences, effective two weeks from today. My last day of work will be September
10, 2021.

Working at Raintree has been a rewarding experience and I have enjoyed my time working with you . I
learned a great deal about working successfully with a team. It was an honor to work with such
dedicated staff who are committed to making Raintree a place to receive compassionate and a high
standard of care.

My decision to leave was difficult, however my family needs me now to take care of them especially my
mother who is going to have her major surgery on September 13, 2021.

I thank you for the training and opportunity to grow with you in my nursing career. I take this chance to
wish all the residents and staff all the best in their future endeavors.

I want you to know that I will do all I can to ensure that my final time in Raintree is as efficient and
seamless as possible.
