I am Ladine Jhalen N. Bacuil a student enrolled at the University of Luzon, presently in Second
Year College and a recipient of the scholarship of CHED. It is informed that I was not able to
claim the check issued for my 2nd semester school year 2019-2021 school fees, for this reason
this has become stale. For this I would like to request the kind consideration of your office that
this be replaced with a new check.
