December, 2021.

Verify spelling of name, date of birth, gender, birth certificate pin number, subjects pursued at CAPE,
email address and phone number.

Any errors in the CAPE slip must be highlighted in RED and the correct information inserted in RED next
to it.

Student must sign the slip.

Parent must sign the slip and write in ID/DP number below signature.

If printing is not possible, please rewrite this information below on a sheet of paper, sign and upload a
picture of it to ALL the email addresses below as well as the form teacher:
