Please accept this letter of intent as official notification that I wish to retire under
Total Permanent Physical Disability (TPPD) effective immediately.

At present, I am suffering Post Traumatic Stress Disorder (PTSD) from the grenade
blast incident I have experience and I have been noticing some changes in many aspects of
my life, both personally and professionally. Because of this, I have become aware that my
disability will require extended treatment and recovery, and I am unsure that my ability to
perform the duties of my present position will return.

I have given considerable thought to this significant decision and it is with a heavy
heart that I will leave the PNP organization which I have proudly served for more than
Twenty One (21) years.

Serving the community and working with my colleagues every day has been one of
the best experiences in my life. May I take this opportunity to thank you most graciously for
the help, support and for allowing me to serve and protect the community I love.

I appreciate your understanding. If there is anything I can do to help during the

transition, please let me know.
