AFFIDAVIT OF LOSS

KNOW ALL MEN BY THESE PRESENTS:

I, ANALYN CALDITO GENSON of legal age, Filipino, married, and resident of

AD 219 F CENTRAL BUYAGAN POBLACION 2601 LA TRINIDAD after
having been duly sworn in accordance with law, hereby depose and state the following:

1. I am owner and holder of the following Truck Ban Pass (TPB) issued by the
City government of Baguio received sometime July 2021 for the following trucks with
Plate numbers:

#36 - Plate No. NGL 2569

#42 – Plate No. CBP 3030

2. I have recently discovered the said item is irretrievably lost and can no
longer be found despite diligent efforts to locate the same;

3. It was lost sometime on January 29, 2022, for the truck with Plate number CBP
3030, while the other truck that has a plate number NGL 2569 was lost sometime
on December 25, 2021.

4. That diligent efforts were exerted to locate, retrieve, repossess, restore, and save
the same proved futile, ineffectual, unsuccessful, and fruitless; and

5. I execute this affidavit to attest to the truth of the foregoing and for whatever
legal purpose it may serve.

IN WITNESS WHEREOF, I have hereunto set my hand this 2nd day of February at La
Trinidad, Benguet, Philippines.
