What I Need to Know

This module was designed and written with you in mind. It is here to
help you master ICT. The scope of this module permits it to be used in many
different learning situations. The language used recognizes the diverse
vocabulary level of students. The lessons are arranged to follow the standard
sequence of the course. But the order in which you read them can be changed
to correspond with the textbook you are now using.
The module is divided into two lessons, namely:
• Lesson 1 – Create an online survey form
• Lesson 2 – Process an online survey data

After going through this module, you are expected to:


1. identify the requirements in creating an online survey using Google
Forms.
2. discuss the steps in creating an online survey.
3. follow the steps on how to process an online survey.

CREATE AN ONLINE SURVEY
1 FORM

Deciding to survey people is easy. But creating a survey can seem hard.
In this module, we help make it simple. There are a lot of choices to make
when you’re writing a survey but following some basic rules can help you
make the right choices every time.

What’s In

What is a wiki?

This is a website that is collaborative in nature. It was originally named


Wiki wiki Web. This was introduced by Ward Cunningham in 1995. This ward
came from the Hawaiian “Wiki wiki” which means fast or swift.

• Charles Sturt Library Wiki
• Tolkien Wiki

What is a blog?

A blog is a type of website on the internet which also goes by the name
of weblog.

What is It

A. Online survey form is a questionnaire that you can use to gather data over
the internet. It is very cheap for the part of the researcher, one whose doing
data gathering and it is very accessible for the part of those who will answer
the survey.

To create an online survey using Google Forms, you must have the
following:

Here are the steps on how to create online survey forms:

1. Navigate to https://docs.google.com/forms/ and click Blank.

By clicking you will be directed to Google forms page. Click + to


create a new form.

2. Name your survey.
You can also add a description. If you want to name the Google
Form for your own reference, click Untitled form in the top left corner
to edit.

3. Tap on Untitled Question and write a question

4. Click Multiple choice.

5. Select an option for how the question will be answered.
For all options except for Short answer, Paragraph, Date and Time,
you'll have to write in options for answers.

Distinct questions in the Google Forms

• Short answer and Paragraph - used for getting essay responses.


• Multiple choice – used when presenting several answers to choose
from.
• Checkboxes allows users to select multiple answers.
• Dropdown gives recipients a field to click that reveals a menu they
will select an answer from.
• Linear scale allows users to answer by selecting a rating from a
range such as 1 to 5.
• Date and Time allow recipients to select a date or time.

6. Click the side menu icons to add to your survey.


• The Plus button adds another question.
• The Tt button lets you add a section title and description.
• The Photo and Video buttons allow you to illustrate your survey.
• The two rectangles icon allows you to break your survey up by
sections.

7. Click the Required switch to make a question mandatory. Click
the duplicate or trash icons to clone or erase the question.

8. Click the Palette icon to change your survey's color or add a photo
to the header.

9. Click the Eye icon to preview your survey.

10. Click the Gear icon to access survey settings.

11. Click Send

If you want to share the survey via a hyperlink, it can be found


by clicking the link icon. To get code for embedding the survey on a

website click the <> icon. You can also share the survey via social
media with the Google+, Facebook and Twitter buttons.

Now that your survey is sent, your audience will have to answer. To
see what your recipients said, click on Responses.

B. Process an Online Survey Data

A summary of the responses will show you the number of responses


to the survey and the results for every question. It will also provide you the
percentage of each question. Aside from that, this will give you a summary in
a form of different graphs, like a bar graph, line graph and pie graph that you
can also use to support your data analysis and results

From the data summary provided by the online survey tool, it would be
easy to analyze it. Here are some points to remember in analyzing the survey
data.

a. View responses
Gather survey responses in a spread sheet saved to your
Google Drive. Open the spread sheet to view the responses.

b. Cross-tabulating and filtering results


Recall that when you set a goal for your survey and
developed your analysis plan, you thought about what subgroups
you were going to analyze and compare.

c. Evaluate the derived numbers

d. Draw conclusions

Weave a story with the collected and analyzed data.
