What I Need to Know
This module was designed and written with you in mind. It is here to
help you master ICT. The scope of this module permits it to be used in many
different learning situations. The language used recognizes the diverse
vocabulary level of students. The lessons are arranged to follow the standard
sequence of the course. But the order in which you read them can be changed
to correspond with the textbook you are now using.
The module is divided into two lessons, namely:
• Lesson 1 – Create an online survey form
• Lesson 2 – Process an online survey data

______4. It is used for getting essay responses.
______5. It allows users to answer by selecting a rating from a range such as
1 to 5.

for each question.

______9. It gives recipients a field to click that reveals a menu they will
select an answer from.

1 FORM
Deciding to survey people is easy. But creating a survey can seem hard.
In this module, we help make it simple. There are a lot of choices to make
when you’re writing a survey but following some basic rules can help you
make the right choices every time.

What is It
A. Online survey form is a questionnaire that you can use to gather data over
the internet. It is very cheap for the part of the researcher, one whose doing
data gathering and it is very accessible for the part of those who will answer
the survey.
To create an online survey using Google Forms, you must have the
following:

2. Name your survey.
You can also add a description. If you want to name the Google
Form for your own reference, click Untitled form in the top left corner
to edit.

5. Select an option for how the question will be answered.
For all options except for Short answer, Paragraph, Date and Time,
you'll have to write in options for answers.

7. Click the Required switch to make a question mandatory. Click
the duplicate or trash icons to clone or erase the question.
8. Click the Palette icon to change your survey's color or add a photo
to the header.

Now that your survey is sent, your audience will have to answer. To
see what your recipients said, click on Responses.

From the data summary provided by the online survey tool, it would be
easy to analyze it. Here are some points to remember in analyzing the survey
data.

Gather survey responses in a spread sheet saved to your
Google Drive. Open the spread sheet to view the responses.
